$24.02/hr - $24.02/hr What You Will Do

Under the supervision of the Case Management Program Manager, provide ongoing engagement, assessment, intervention, and evaluation for youth on the individual case load. This includes youth intakes, assessments, crisis phone counseling, client data collection, documentation, file maintenance, and follow-up care. Assist with general upkeep of the case management program, including regular program shift coverage, milieu management, crisis intervention, reporting maintenance issues, and ensuring that the program is facilitated in a manner that meets Urban Peak’s mission. Work with youth ages 12-24 who are enrolled in Urban Peak’s programming and experiencing homelessness. Address varying client acuity with goals related to mental health, substance use, education, employment, and housing. Ensure interactions, interventions, and environments are trauma informed and emphasize the physical and emotional safety of youth and staff. Who You Are

High school diploma or equivalent with a minimum of five (5) years full-time experience working with high-risk youth, OR a Bachelor’s Degree in human services or related field with at least three (3) years of experience working with youth, OR a Master’s Degree in social work, psychology or related field with at least two (2) years of experience working with youth. Certified Addiction Counselor (CAC) preferred (differential pay of +$2/hr offered). Lived experience reflective of the youth served encouraged, but not required. Ability to connect with a diverse youth population in a culturally responsive manner. Understanding of trauma-informed care and harm reduction principles, with ability to implement these approaches. Ability to handle multiple tasks and adjust priorities based on agency and client needs, in environments that may include interpersonal conflict and chaos. Confidence in timely and accurate data collection and maintaining thorough client files (this position will utilize Salesforce). Skill in operating office equipment and software (Microsoft Word, Excel, PowerPoint, Outlook, Teams) and telephones. Skill in mediating and de-escalating in sensitive situations. Able to make sound decisions while maintaining confidentiality. Ability to be insurable as a driver on Urban Peak’s auto liability policy to transport youth using Urban Peak vehicles and approved personal vehicles. Ability to pass federal and state background checks and a pre-employment TB screening. Case Management

Manages a case load of up to 30 clients. Meets with assigned youth weekly to create goals, review progress, and provide support and accountability. Provide personal attention to youth, supporting their social, emotional, and intellectual development. Assist clients in moving through Urban Peak programs by helping them apply for Neighborhood and Housing programs in a timely manner. Provide guidance based on trauma-informed care, positive youth development, and harm reduction principles. Assist youth and others in facilitating safe and stable housing. Collaborate with other agencies for service delivery, staying current on resources and developments in this area. Cultivate a shame-free case management relationship where youth can discuss substance use, sexuality, gender, religion, mental health, neurodivergence, and other identities without judgment. Conduct and/or participate in circles of support or restorative conversations as needed. Be a positive role model by communicating consistently and showing up on time for case management meetings. Attend required staff meetings and weekly supervision with client updates. Work with parents, legal guardians, or family members towards reunification when appropriate. Milieu Supervision

Provide staff oversight of the shelter milieu during weekly program shifts (4-5 hrs.), holiday shifts, and on-call shifts. Provide services through a trauma-informed approach to ensure physical and emotional safety. Offer programming that supports the building of healthy relationships as needed. Coordinate with kitchen staff and volunteers to ensure meals are prepared safely and efficiently. Assist youth in maintaining a clean environment and completing daily chores. Maintain a welcoming presence and support youth who enter the program through intakes, tours, and bed assignments. Provide crisis intervention and de-escalation support, including handling verbal and physical altercations and phone counseling as needed. Documentation

Document all services in Salesforce, including intakes, assessments, case notes, treatment plans, and both individual and bulk services. Enter client documentation in accordance with data quality standards. Obtain Releases of Information for all communications with external supports on behalf of clients. Document significant events and client updates in the Communication Log. Maintain clear, effective communication with other staff regarding clients. Ensure client confidentiality through secure storage of information. Complete thorough intakes to establish trust and comfort, assess level of need, and make recommendations and referrals.
