Description
The Home Health RN Case Manager is responsible for coordinating and providing skilled nursing care to patients in their homes. This role oversees the overall plan of care, ensures compliance with regulatory requirements, and collaborates with patients, families, physicians, and the interdisciplinary team to achieve positive clinical outcomes.
Key Responsibilities
Perform comprehensive patient assessments, including physical, psychosocial, and environmental factors.
Complete OASIS including Start of Care, Resumption of Care, Recertification and Discharge OASIS.
Develop, implement, and update individualized Plans of Care (POC) in collaboration with physicians and the interdisciplinary team.
Provide direct skilled nursing care, including wound care, medication administration, IV therapy, disease management, and other nursing interventions.
Serve as the primary case manager, coordinating care with therapists, aides, social workers, and physicians.
Monitor patient progress, adjust care plans, and communicate changes to the physician promptly.
Educate patients and caregivers on disease processes, self-care, medication management, and safety.
Ensure accurate, timely, and compliant documentation in the electronic health record (EHR).
Participate in case conferences, quality improvement initiatives, and agency staff meetings.
Ensure compliance with Medicare, state, and agency regulations and policies.
Advocate for patient needs while promoting independence and quality of life.
Provide on-call support as assigned, including after-hours and weekend visits.

Proudly serving 19 counties in Indiana, we are a preferred provider for the BHI, Inc. Communities in Carmel, Indianapolis, and Zionsville. We offer skilled nursing, physical therapy, occupational therapy, and speech therapy to patients in the comfort of their homes. Additionally, we provide private duty services in the home through our Personal Assistant Program, which is billed at a private pay hourly rate. We are a Medicare participating provider as well as a Medicaid participating provider, accepting Home and Community-Based Service Waivers and PA.
Requirements
Current Registered Nurse (RN) license in the state of Indiana.
Minimum of 1–2 years of nursing experience (home health or acute care preferred).
Knowledge of OASIS documentation and Medicare home health regulations preferred.
Strong clinical assessment, critical thinking, and problem-solving skills.
Excellent organizational and communication skills.
Ability to work independently and manage a caseload effectively.
Valid driver’s license, reliable transportation, and proof of auto insurance.
Travel between Carmel, Zionsville, Anderson, and Indianapolis.
Physical Requirements
Ability to lift, transfer, and reposition patients as necessary.
Frequent driving to patient homes.
Ability to stand, walk, bend, and perform physical nursing care.
