Rochester Regional Health is seeking a dedicated and highly motivated Physician Assistant (PA) to join the Pulmonary team in the Wilson Multicultural Medical Clinic. This is a hybrid role supporting both inpatient and outpatient Pulmonary care with a primary focus on COPD management. The PA will also have the opportunity to contribute to our Lung Cancer Screening Program (LCSP), supporting early detection and prevention efforts as part of our transition toward a value-based care delivery model. Interacts on a frequent, regular and daily basis with patients, family members, medical/social/public services support, regulatory agencies and/or payor agencies. Medication & Orders. Prescribe medication, write orders, perform procedures and respond to emergency and/or immediate condition changes of acute, critically ill patients. Documentation. Obtain complete, detailed and accurate patient histories, review patient records, perform physical exams, record all pertinent data to develop comprehensive medical status, diagnosis and clinical treatment plan for all assigned patients. Manage COPD patients across both inpatient and outpatient settings, including follow-up care, medication management, and patient education. Participate in Multidisciplinary Rounds and care coordination. Collaborate with physicians and other team members to deliver high-quality, patient-centered care. Lung Cancer Screening Program (LCSP) contribution up to 2 days per week. This component may also be performed remotely (optional), depending on operational needs. Assist with patient outreach, screening eligibility reviews, shared decision-making conversations, and follow-up coordination for LCSP participants. Support Rochester Regional Health’s transition toward value-based care through proactive chronic disease management, engagement, and quality improvement initiatives. Education : BS: Physician Assistant (Required) Licenses / Certifications : PA-C - Certified Physician Assistant - National Committee on Certification of Physician Assistants (NCCPA), RPA - Registered Physician Assistant - New York State Education Department (NYSED) Physical Requirements : M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
