About the Role
As a Senior Practitioner in the Leaving Care 16+ Team, your primary focus will be to safeguard and promote the well-being of vulnerable young people transitioning from care to independence. You will deliver high-quality social work practice, supporting young people to achieve their full potential while ensuring that their needs are met effectively, whether they remain within their family (if safe to do so) or live independently. You will actively participate in effective partnership working with statutory and voluntary organisations, engaging collaboratively to deliver a holistic service to young people. Your role will comply with legislation, statutory guidance, and local policies and procedures, ensuring excellence in practice as outlined in the Professional Capabilities Framework (PCF) at the Experienced Social Worker level and the Knowledge and Skills Statement (KSS) for child and family practitioners. Key Responsibilities
Provide high-quality social work practice to support young people transitioning from care to independence. Develop and implement pathway plans that reflect the aspirations, needs, and circumstances of young people aged 16–25. Promote young people’s welfare, safeguarding their safety, and ensuring they are empowered to make informed decisions. Support young people in accessing education, training, employment, health care, and accommodation services. Undertake comprehensive assessments to identify young people’s strengths, needs, and risks, ensuring that appropriate safeguarding measures are in place. Respond promptly to safeguarding concerns, working within statutory and organisational frameworks to protect young people from harm. Work collaboratively with families, carers, and professionals to address and reduce risks to young people. Engage with a wide range of statutory and voluntary partners to ensure that young people receive coordinated support tailored to their needs. Act as an advocate for young people, ensuring their voices are central to planning and decision-making processes. Represent the team in multi-agency meetings, including child protection conferences, pathway reviews, and court proceedings, providing expert input and high-quality reports. Requirements
Qualifications : A recognised Social Work degree (e.g., BA/MA in Social Work, DipSW, or equivalent) Registration : Current registration with Social Work England Experience : Extensive post-qualification experience in children’s social work, particularly in child protection, safeguarding, and court work Supervision : Previous experience supervising or mentoring social workers is essential Knowledge : In-depth knowledge of relevant legislation, policies, and best practice guidance in children’s social care, including safeguarding and risk management Skills : Strong leadership, decision-making, and communication skills, with the ability to manage a complex caseload while supporting and developing colleagues Other : A valid driving licence and access to a vehicle for work purposes are desirable Why Join Us?
