Full Time Physical Therapist Case Manager – Home Health. Anticipated Territory: Folsom Area. Requires being in the field visiting patients in their homes. Remote/work-from-home positions are not available unless specifically noted. Base pay range $120,000.00/yr - $135,000.00/yr Wage information

Perform an evaluation in the home and develop a care plan; collaborate with Physical Therapy Assistants (PTAs) to implement the plan. Help patients with mobility and regaining independence; restore function and minimize disability through prescribed physical therapy programs. Evaluate and treat patients; supervise PTAs per agency and regulatory standards; coordinate with patients’ physicians to ensure quality care. Document progress and report patient status to the medical team; participate in interdisciplinary collaboration. What you’ll do as a Home Health Physical Therapist Case Manager
Provide skilled services to evaluate and address home-bound patients’ needs so they can live safely at home. Work with other clinicians on the case to support patient goals. Qualifications
Current unrestricted CA Physical Therapist License Current BLS/CPR Certification Valid CA Driver's License Knowledge of local community resources; understanding of development, behavior, and culture in health care Knowledge of state and federal homecare regulatory guidelines (preferred) Home health care experience highly preferred; experience with high-acuity patients and interdisciplinary teams is desirable Case Management fundamentals (highly desirable) Self-directed, empathetic, critical thinker with outstanding interpersonal and leadership skills Organized, able to manage responsibilities and priorities Benefits & Perks

Wage range: Physical Therapist, Level 1: 120,000.00 - 135,000.00 with up to 10K annual bonus (paid quarterly). Starting pay is commensurate with relevant experience. 21st Century offers bonus plans, health benefits, and 401k (4% match). Most PT Case Managers are hired as PTIs; promotion to PTII with higher load and autonomy includes a minimum salary of 135,000 plus 20K bonus (paid quarterly).
