Opportunity Information Sutter West Bay Medical Group (SWBMG) is seeking a skilled and collaborative Nurse Practitioner or Physician Assistant to join our Valve Clinic team in Oakland, California. This role supports a multidisciplinary group of specialists — including Advanced Heart Failure (AHF), Structural Heart/Interventional Cardiology (IC), and Cardiothoracic Surgery (CTS) — in the evaluation and management of patients with valvular heart disease across both outpatient and inpatient settings.
About The Opportunity This position offers the opportunity to work in a dynamic, team-based environment focused on optimizing care for patients with complex valve pathologies. The APC will play a central role in coordinating care, managing clinic visits, supporting inpatient consults, and facilitating multidisciplinary collaboration. While direct provider involvement in valve pathology is minimal, the APC will be instrumental in ensuring continuity and quality of care.
Key Responsibilities
Provide patient care in both outpatient and inpatient settings, including initial assessments, follow-up visits, and hospital-based consults
Coordinate care across specialties including Advanced Heart Failure, Interventional Cardiology, and Cardiothoracic Surgery
Assist with pre‑ and post‑procedural planning and education
Conduct telehealth visits and manage patient communications via the Epic platform
Participate in multidisciplinary valve conferences and contribute to care planning
Educate patients and families on valve disease, treatment options, and care pathways
Support quality improvement and patient experience initiatives
Valve Pathologies Managed
Degenerative Mitral Regurgitation (MR): Managed by Interventional Cardiology and Cardiothoracic Surgery
Functional Mitral Regurgitation (MR): Managed by Interventional Cardiology and Advanced Heart Failure
Tricuspid Regurgitation: Managed by Interventional Cardiology and Advanced Heart Failure; once euvolemic, by Interventional Cardiology and Cardiothoracic Surgery
Aortic Stenosis and Regurgitation: Managed by Interventional Cardiology and Cardiothoracic Surgery

Education Graduate of an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program.
Qualifications
Graduate of an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program
Licensure & Certifications
Current California NP or PA license
DEA Controlled Substance Registration Certificate (CSRC)
Current BLS certification
Current ACLS certification
Additional certifications as required by department
Experience
Prior experience in cardiology, heart failure, or structural heart disease preferred
New graduates with a strong interest in valvular heart disease and multidisciplinary care encouraged to apply
