Opportunity Information: Sutter West Bay Medical Group (SWBMG) is seeking a skilled and collaborative Nurse Practitioner or Physician Assistant to join our Valve Clinic team in Oakland, California. This role supports a multidisciplinary group of specialists—including Advanced Heart Failure, Structural Heart/Interventional Cardiology, and Cardiothoracic Surgery—in the evaluation and management of patients with valvular heart disease across outpatient and inpatient settings. This position offers the opportunity to work in a dynamic, team-based environment focused on optimizing care for patients with complex valve pathologies. The APC will coordinate care, manage clinic visits, support inpatient consults, and facilitate multidisciplinary collaboration. Direct provider involvement in valve pathology is minimal, but the APC will be instrumental in ensuring continuity and quality of care. Responsibilities

Provide patient care in both outpatient and inpatient settings, including initial assessments, follow-up visits, and hospital-based consults Coordinate care across specialties including Advanced Heart Failure, Interventional Cardiology, and Cardiothoracic Surgery Assist with pre- and post-procedural planning and education Conduct telehealth visits and manage patient communications via the Epic platform Participate in multidisciplinary valve conferences and contribute to care planning Educate patients and families on valve disease, treatment options, and care pathways Support quality improvement and patient experience initiatives Valve Pathologies Managed

Education

Graduate of an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program Licensure & Certifications

Current California NP or PA license DEA Controlled Substances Registration Certificate (CSRC) Current BLS certification Current ACLS certification Additional certifications as required by department Experience

Prior experience in cardiology, heart failure, structural heart disease, or related specialty required New graduates with a strong interest in valvular heart disease and multidisciplinary care are encouraged to apply Medical Group Details
