Overview
Position: Physician Assistant or Nurse Practitioner (Full-Time) Location: Baraboo, WI Pay Range: $118,000 - $149,000 per year. Salaried position (Based on Experience and FTE). Additional compensation: Sign-on bonus. Provide full-spectrum primary care for patients of all ages in our advanced direct primary care model. Evaluate, diagnose, and treat both acute and chronic conditions while emphasizing prevention, education, and long-term health outcomes.
Build meaningful patient relationships with extended visit times (30-60 minutes) and proactive outreach to support personalized, trust-based care.
Collaborate within a physician-led, multidisciplinary team to deliver coordinated care, participate in case reviews, care planning, and integration with specialty and community partners.
Practice preventive and lifestyle-focused medicine, guiding patients through evidence-based lifestyle interventions and integrating preventive screenings and wellness coaching.
Manage acute and urgent care needs, including minor procedures and point-of-care diagnostics appropriate to your skill set.
Develop and manage chronic disease care plans for conditions such as diabetes, hypertension, obesity, mental health, and musculoskeletal disorders, leveraging technology to monitor progress.
Leverage Astia’s EMR, population health dashboards, and AI-assisted tools to track patient engagement and identify care gaps.
Contribute to continuous improvement through clinical projects, workflow optimization, and quality initiatives.
Grow into leadership roles over time, mentoring providers and expanding service lines where applicable.
Perform employment-related exams (pre-employment physicals, return-to-work evaluations, DOT/Non-DOT exams) as required.
Injury evaluation and follow-up for work-related injuries, coordinating return-to-work plans with employers and safety teams.
Collaborate with employers and safety partners, maintaining medical confidentiality and supporting workplace health initiatives.
Participate in health screenings, immunization campaigns, and wellness initiatives for local employer groups.
Maintain compliance and documentation standards using Astia’s EMR and data tools.
What We’re Looking For
Licensed Physician Assistant or Nurse Practitioner in Wisconsin with at least 2 years of experience in primary care, family medicine, emergency medicine, or urgent care.
Strong collaborative communicator with a team-first mindset.
Commitment to high-quality, patient-centered care.
Active or eligible DEA registration prior to start.
Excellent clinical judgment, communication, and problem-solving skills.
DOT physicals: Federal Med certification (Certified or within 3 months of start date).
