Advanced Practice Provider NP/PA - Geriatric Medicine
Department: Pace Clinic - Geriatric Medicine. Responsibilities : In collaboration with a physician, APPs provide for the expansion of individualized primary healthcare services by diagnosing and treating certain acute or chronic health conditions. Obtain patient history and performs assessment via observation, interview and examination. Order, perform and interpret diagnostic studies. Perform preventative health assessments, screening, immunizations and patient care. Provide direct treatment and management of health conditions via referral to other healthcare providers or community resources. Counsel and educate patients and their families/caregivers concerning preventative health, treatment options and community resources. Document delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy. Job Duties : APP works mostly independently but under the direction of a supervising physician. Call will be 1:6 but currently is 1:4. Will provide services at Ascension Living Flint, MI clinic, and could include rounding on inpatients from time to time but not very often. Typically see 4-6 patients in a clinic setting. About PACE : The Program for All Inclusive Care (PACE) is an alternative to nursing home care and – through an interdisciplinary care team of physicians, nurses, social workers, therapists, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community. Aged 55 years old or older. Live in a PACE provider’s service area. Certified by the state as meeting the need for the nursing home level of care. Able to live safely in the community with the help of PACE services at the time you enroll. While there are no financial eligibility requirements, PACE programs are covered by Medicare and Medicaid for eligible individuals. PACE services are also available on a private pay basis. Requirements : Licensure / Certification / Registration: BLS Provider, Nurse Practitioner, Registered Nurse. Education: Master's degree of Nursing.
