Groundwork Behavioral Health Servic
· Assist persons served by implementing structured interventions outlined in the Person-Centered Plan, including:
Individual counseling and support;
Group counseling and support;
Family counseling, training, or support;
Biochemical assays to identify recent drug use (e.g., urine drug screens);
Strategies for relapse prevention, including community and social support systems;
Life skills development;
Crisis contingency planning;
Disease management;
Support activities adapted for persons with physical disabilities or co-occurring disorders such as mental illness and substance abuse/dependence or mental retardation/developmental disability.
· Provide first responder crisis response.
· Ensure a supportive, therapeutic relationship between staff, persons served, and their families, implementing interventions as outlined in the Person-Centered Plan.
· Promote principles of recovery by emphasizing self-determination.
· Assist persons served by researching and providing access to natural and community supports.
· Support persons served in understanding and exercising their rights.
· Deliver support services in a safe, therapeutic manner, free from abuse, neglect, and exploitation.
· Document all information from health-related appointments.
Performance Expectations:
Serve as a role model by conducting oneself professionally and ethically, including promptness, effective communication, maintaining boundaries, representing the agency professionally, and complying with Federal Anti-Kickback and Fraud Laws.
Maintain therapeutic boundaries with consumers, families, and guardians.
Be familiar with HIPAA regulations related to confidentiality.
Understand client rights.
