Advanced Practice Provider (NP/PA) - Medicine Cardiology

Overview

UNC Cardiology has a great team of APPs, and we are looking to add to it! Both NPs and PAs are welcome to apply to any of our APP positions. This position is referred to as our ACPW (Alternate Care PathWays) APP. ACPWs quickly facilitate and move patients to safe, less expensive sites of service, while still delivering high quality care and avoiding potential risks associated with hospitalization. In practical terms, some patients who present at the ED (Emergency Department) and need to be seen quickly may not require hospitalization. These are patients with specific, moderate acuity conditions who are medically stable. This APP plays an important role in reducing unnecessary inpatient admissions and readmissions. This APP will receive referred patients/appointments from the UNC EDs at Chapel Hill or Hillsborough hospitals, for the purpose of seeing them in a timely manner (within 72 hours) in one of our Cardiology clinics (at Eastowne in Chapel Hill or at UNC’s Hillsborough Medical Office Building). Because the number and frequency of patients referred in this way is variable, this APP will also see other Cardiology patients—e.g., Chest Pain, AF transition, hospital follow-ups, and potentially others as appropriate. The APP may also spend a relatively small portion of time on quality improvement related to access (e.g., meetings, coordination with colleagues). Our clinic APPs back each other up, so the successful candidate should be flexible with giving and receiving help as needed. Each APP has a supervising physician but also works independently, with their own clinic template and schedule. The clinic schedule is weekday daytime hours (Mon-Fri, 8-5).
Summary : A Nurse Practitioner functions as part of the interdisciplinary healthcare team in accordance with privileges approved by the credentialing committee to provide high quality, cost effective care to patients within the APP scope of practice in collaboration with a supervising physician. The APP reflects the mission, vision, and values of the organization, and complies with all relevant policies, procedures, guidelines and other regulatory and accreditation standards.
Responsibilities
Clinical – Obtain relevant health and medical history, perform physical assessment, review and interpret diagnostic tests/results, develop differential diagnoses, and implement treatment plans including prescribing/ordering appropriate evidence-based pharmacological agents in line with departmental guidelines and privileging. Schedule follow-up management and contribute to hospital initiatives to improve patient care. Conduct preventive health screening based on age and patient needs. Maintain clear, comprehensive records of patient care. Maintain knowledge of general disease/pathologic processes and specialty literature. Develop differential diagnoses, revise plans as patient needs change, prioritize work, and assist staff to ensure prompt and efficient patient flow. Contribute to productivity standards and participate in system improvements. Perform medical procedures as appropriate per guidelines and responsibilities. Education – Educate patients and families about medical conditions, treatments, and follow-up. Collaborate with clinicians, staff, and students to promote continuity of care. Develop educational materials based on learning needs. Provide instruction to physicians, housestaff, nursing, ancillaries and/or students. Pursue identified self-learning needs and attend education programs as appropriate. Leadership – Provide instruction to physicians, housestaff, nursing, ancillaries and/or students (e.g., precepting, lectures, rounds, and staff meetings). Engage in community/network outreach programs and lead or participate in systems improvements to optimize healthcare delivery. Professional – Interact with patients and families respectfully and in a timely manner. Work collaboratively with team members and staff to meet patient care needs. Respond quickly to patient needs, accept responsibility for outcomes, and share workload as needed. Quality/Research – Engage in daily quality improvement, identify and implement process improvements, consult with management on clinical and system issues, lead interdisciplinary teams, update protocols to reflect evidence-based care, influence policy-making to improve patient care, and develop new clinical programs or services as appropriate.
Education, Licensure, and Experience
Education Requirements: * Completion of an accredited graduate-level Advanced Practice Provider (APRN or PA) program. Licensure/Certification Requirements: * Licensed as an Advanced Practice Provider (e.g., Nurse Practitioner, Physician Assistant, Clinical Pharmacist Practitioner, Certified Nurse Midwife, Certified Registered Nurse Anesthetist) in the state of North Carolina. * Board certification if required by the Department. DEA. Professional Experience Requirements: * No prior experience required. Knowledge/Skills/Abilities: * Demonstrated clinical competency. Strong leadership and interpersonal skills. Ability to work independently and as part of a team. Strong written and verbal communication and analytical skills. Ability to document accurately for billing and patient care.
