The
Health Information Scanning Analyst
is responsible for compiling, organizing, and analyzing patient medical records for Hospital Inpatient and Outpatient services to support network goals for accuracy, timeliness, and completeness, ensuring compliance with CMS, NJDOH, TJC, and HMH policies. Responsibilities
Retrieve paper records of all discharged patients from nursing units across campuses. Ensure all discharged charts are accounted for, utilizing the Chart Reconciliation work queue in Epic. Organize and assemble charts, removing non-scannable documents and ensuring correct patient information on each page. Prepare medical records for scanning, label documents, and remove staples and tape. Scan documents into the legal medical record, ensuring data integrity. Perform quality control checks on scanned images for accuracy and re-scan if necessary. Investigate, verify, and correct patient information using hospital applications to maintain record accuracy. Assist with merging potential duplicate records and complete merges as per policy. Review and analyze HI deficiencies in Epic with high accuracy, including responsible physician identification. Monitor and review Chart Deficiency work queues for timeliness and accuracy. Notify providers of delinquent records and assist in completing deficiencies. Generate Release of Information reports and assist with MyChart issues. Request archived records and handle medical record requests professionally. Adhere to organizational competencies and standards of behavior. Qualifications
Required:
High School diploma or GED, good organizational and communication skills, computer literacy, proficiency in Microsoft Office and Google Suite. Preferred:
Experience in health information or healthcare fields. Additional Details
