Summary
The HIM Analyst is responsible for reviewing the medical record for completeness, timeliness, and accuracy. The review identifies whether all required documentation is present and that all entries have been authenticated before the record is filed as complete. The Analyst assigns identified deficiencies to the responsible provider(s) for timely completion and performs re‑analysis to ensure deficiencies are resolved accurately.
Duties and Responsibilities
Review medical record documentation in electronic or paper records, create charting deficiencies in the deficiency management system, and assign those deficiencies to the appropriate provider(s). Scan paper records to create a single electronic encounter for the patient visit.
Use the EMR system via reports and work queues to support record completion, coding, release of information, and overall management of the legal health record.
Serve as an information resource, responding to requests for general and medical record information received by phone, e‑mail, fax, or in person. Retrieve records for patient care, studies, committees, and other requests, working with departments such as Registration and Patient Accounts to resolve issues.
Identify duplicate medical record numbers, ensuring each patient and number is unique and correct to safeguard the integrity of the Master Patient Index and provide an accurate database for all applicable systems.
Actively manage analysis‑specific work queues, both electronic and manual, to ensure timely analysis and chart completion.
Navigate and utilize basic computer applications and multitask in a fast‑paced, ever‑changing health‑care environment.
Adhere to established company standards/policies and system workflow guidelines to add and re‑assign accounts to appropriate work queues for processing.
Identify documents needing re‑scan due to poor image quality and route them to the appropriate work queue for processing.
Contact physicians and their office staff to facilitate timely medical record completion.
Address appropriate questions related to medical record completion to physicians and other providers.
Meet or exceed hospital productivity standards.
Meet or exceed hospital quality standard of 95% or greater.
Assist in maintaining all equipment and supplies required to provide services in an optimal manner.
Promptly report issues and trends that do not comply with facility or corporate policies/standards.
Assist in cross‑training other employees as necessary.
Exhibit interpersonal skills necessary to exchange accurate information with internal and/or external customers on a frequent basis.
Responsible for scanning patient’s medical records into the document imaging system for timely coding and billing purposes.
Prepare weekly delinquent chart reports.
Perform monthly charge reports (open chart reviews, 100% chart reviews, etc.).
Provide physician education as needed.
Be able to work independently.
Perform other duties as assigned.
Physical Movements Sitting, reaching, bending, stooping, and stretching.
Education & Experience
High school diploma or equivalent required.
Bachelor’s degree in Health Information Management preferred.
One (1) year experience in a medical office environment required; prior experience in medical record analysis or an HIM department strongly preferred.
Understanding of HIPAA laws.
Ability to decipher physician handwriting.
Proficient computer skills.
Possess strong written and verbal communication skills.
Preferred experience with electronic medical record systems – CPSI preferred.
