Job Summary:
Advanced Practice Provider
will serve as a Full-Time APP, providing care to patients within the
Joslin Diabetes Center
with Upstate’s
Nappi Wellness Institute . The APP will provide adult diabetes management in the outpatient setting.
Responsibilities include:
Providing expert level clinical care & coordination of care via a full range of services that will improve quality of patient care assessing & evaluating patients.
Ordering appropriate laboratory testing & diagnostic studies, arranging for referrals, testing, consultation & therapeutic services.
Maintaining outstanding relationships with families, referring physicians & all members of the health care team.
Minimum Qualifications: Master’s Degree as a Nurse Practitioner or Physician Assistant.
NYS Licensed/Registered NP or PA.
Current National Certification consistent to the clinical practice.
NYS licensed/registered NP or PA required. Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire).
CPR-BLS Certification required at the time of appointment.
Preferred Qualifications: Diabetes Management.
Experience in an Ambulatory setting.
Work Days: Monday - Friday, Days
