Advanced Pharmacy Practice Provider - 40 hrs/wk - Bingham Farms

Overview GENERAL SUMMARY: Under the direction of Ambulatory Clinical Pharmacy Services, the Advanced Pharmacy Practice Provider (APPP) provides oversight and leadership for appropriate medication use within their region of the health system. The APPP works closely with the medical and administrative leadership as well as the primary care providers to develop and implement strategies which continuously improve the quality of patient care at the best value for the patient and our health system. The APPP serves as an essential member of the ambulatory care team to achieve and maintain improved patient health outcomes, enhanced patient experience, reduced health care costs, and improved provider work life balance. The APPP practices at the top of licensure and with expanded scope of practice through a collaborative practice agreement (CPA). The APPP supports population health strategies and value-based care initiatives. The APPP coordinates care with the provider, care team, and other pharmacy services across Henry Ford Health. The APPP participates in ongoing research, scholarship, quality initiatives, projects, committees, policy development, and formulary management strategies. The APPP assists in identifying opportunities to streamline patient care, improve the provision of pharmacy services, decrease waste, and rework in the region, and improve drug utilization. The APPP will be instrumental in guiding the further development of other pharmacists on the health care team.
