Position: Community Health Outreach Specialist - Winston Salem, NC

At Monogram Health, we are transforming care for high-risk patients with kidney disease and other polychronic conditions, and as a Community Health Outreach Specialist, you are at the heart of that mission. In this role, you will engage face-to-face with Monogram-eligible patients who could benefit from our services but are not currently enrolled in our program. By educating patients on Monogram, answering their questions, and building their trust, you’ll help connect them to life-changing care. Your outreach has the power to dramatically improve someone’s health, well-being, and quality of life, making every visit an opportunity to make a meaningful difference. The Community Health Outreach Specialist reports directly to a Regional Engagement Manager and is responsible for engaging with Monogram’s attributed patients and their families in their homes. To do so, the Community Health Outreach Specialist will be required to routinely visit patient homes and introduce them to Monogram Health. In all these situations, the Community Health Outreach Specialist will need to be empathetic, inquisitive, and resourceful to identify and solve the patient’s unique needs. This person will spend the vast majority of their time on the road meeting with patients in and around their community. The Community Health Outreach Specialist will need to maintain a close working relationship with their local Monogram Pod, in order to coordinate patient care in real-time. Roles and Responsibilities

Conduct daily in-person outreach to high-risk, often hard-to-reach patients by visiting their homes to educate them in person about the Monogram Health program. Conducts pre-visit research to better understand each patient’s background and potential needs Leverages internal route-finding tool to optimize daily member outreach plan. Proactively schedules and completes at least 10 home visits daily Clearly and compassionately communicate the benefits of Monogram’s care model, answer questions, handle objections, and encourage patient enrollment Ability to engage and build rapport with patients, family and caregivers of diverse socio-economic and cultural backgrounds with multiple chronic conditions and vulnerabilities Maintain a close working relationship with the local Pod to facility warm handoffs to Monogram clinicians, and otherwise meet Monogram’s needs in the community Ability to collaborate effectively with Monogram colleagues and make intelligent real-time decisions to improve patient care and outcomes Facilitate a high completion rate of scheduled visits Maintain accurate records of outreach activity in Salesforce and collaborate closely with clinical and operational teams Evaluates patient’s progress and effectiveness of resources or services, making necessary and appropriate changes based upon patient’s status Position Requirements

Based in or near Winston Salem, NC with daily travel to patients’ homes across the city and surrounding areas. Heavy Travel – requires daily driving to patient homes Valid driver’s license, reliable transportation and auto insurance required. 2+ years’ experience working in healthcare, sales, outreach, engagement, and/or enrollment campaigns Strong communication skills with the ability to explain complex programs in simple, compassionate terms. Exceptional soft skills including empathy, adaptability, resilience, cultural sensitivity, and comfort handling rejection.
