Under administrative guidance, the Clinical Pharmacist – Transplant will provide comprehensive pharmacy care services relating to transplant, including the review of appropriate drug therapy, consultations with physicians for each transplant recipient, and reviewing dose and inter-patient variables in all transplant patients. This pharmacist will provide thorough medication education to all transplant recipients prior to discharge from the hospital and will actively participate in daily rounds with the Transplant team. Attend daily medical rounds with surgeon and nurse practitioner. Coordinate treatment plan with other caregivers. Prescribe interventions and communicate needed changes to ensure the appropriate individualized plan of care is implemented. Apply knowledge of age-specific needs (pediatric, adolescent, adult and geriatric) to related plan of care interventions and patient responses to care. Provide medication counseling and education to all new transplant patients and provide physician support in hospital and clinic settings. Qualifications

The expertise and experiences you’ll need to succeed: PharmD from college of pharmacy Licensed Pharmacist (RPH) Experience in organ transplantation 1+ year of experience as a clinical pharmacist or completion of a Transplant residency PGY1
