Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in their homes and helping bridge gaps in care. You will play a critical role in building trusted relationships to make people healthier by bringing flexible, comprehensive and personalized health evaluations to people at their front doors. Responsibilities
Bring your heart into every visit, joining a national network of purpose-driven clinicians dedicated to improving health outcomes Visit millions of people nationwide with the goal of connecting them to the right care for their unique needs Work as a clinician at Signify Health, fulfilling the requirements of the specified role Be flexible to travel locally and within licensed states, as needed Conduct virtual visits as needed Provide all Signify Health services as indicated including Diagnostic & Preventive Services (e.g., spirometry, DEE, bone density screening) Work Environment
Primary workplace is the private residence of our members. Environments are diverse and may present various conditions. Candidates must be able to perform duties safely in homes that may contain pets, smoke or other airborne irritants. Required & Preferred Qualifications (NPs and PAs)
Active, unrestricted license(s) in coverage area(s) Board certification (required) Multi-state licenses (preferred) Able to cover primary locations: Queens and surrounding area Compensation and Benefits
The pay range represents base pay per In-Home Health Evaluation and may vary with daily volume. Eligible for other reimbursement opportunities, including pay per completed Diagnostic and Preventive Services as ordered, in addition to base pay. Details provided during selection. Annual full-time salary range: $95,738.00 - $206,206.00 A Comprehensive benefits package for full-time employees including health, dental and vision insurance; paid time off; annual CEU allowance; 401(k); supplies and other perks; malpractice coverage Eligible for additional benefits such as wellness programs, dependent resources, tuition assistance and other programs depending on eligibility Allied Information
Anticipated Weekly Hours: 40 Time Type: Full time Pay Range: The typical pay range for this role is $95,738.00 - $206,206.00. The actual base salary offer depends on experience, education, geography and other factors. Additional Details
