JOB SUMMARY:

The QIDP ensures individuals with developmental and intellectual disabilities receive continuous active treatment in accordance with Individual Support Plans (ISPs). They are also responsible for providing counseling, case management, and structured behavior programming to people with disabilities receiving day program services. Additionally, QIDP is responsible for the implementation of rules and regulations as required by licensing entities. JOB FUNCTIONS: (include but not limited to the following) Participate in both annual and semi-annual personal plan meetings for all residents on caseloads. Ensure implementation strategies, consent forms, rights restrictions, assessments, and participant, guardian, and other required parties complete other required documentation timely and signed. Complete monthly case notes, review goal trackings, complete revisions as required. Assess individual capabilities and integrate appropriate therapeutic services. Ensure environmental and individual supports (i.e. assistive devices). Ensure that all service plans and files comply with state licensing standards. Oversee direct support personnel in assigned facilities, including direct supervisor of Residential Assistant Coordinators. Develop a positive relationship with Guardians, ISCs, and other members of the IDT through prompt and consistent communication. Assist and oversee weekly schedules in assigned facilities. Assist Residential House Supervisor in emergency coverage as needed. Ensure all medical appointments and follow-ups are scheduled in a timely schedule. Participate in all assigned committees including but not limited to, Human Rights Committee, Behavior Management Committee, and Medical Review Committee on a quarterly basis. Ensure all assigned individuals are enrolled in benefits such as SNAP, Medicare, and Medicaid and renewals are timely completed. Review EOM paperwork as submitted by the Residential House Supervisor. Communicate with families regarding incidents, policy changes, status of program, and potential advocacy efforts. Prepare program reports for Management as requested. Maintain a high level of professionalism. Support assigned homes Monday-Friday after hours on an on-call basis. Participate on a rotating on-call schedule for weekends as assigned. Other duties as assigned. QUALIFICATIONS AND REQUIREMENTS: Bachelor’s degree in general or special education, Psychology, Sociology or a related human services field; One to two years of prior experience with the Intellectual and Developmental Disability (IDD) population; Knowledge of Applied Behavior Analysis Principles; Passion and commitment working with adults with intellectual and developmental disabilities; Successful completion of First Aid/CPR certification required within four months of hire or current certification; Current, valid Illinois driver’s license and proof of insurance required; Reliable means of transportation required; Fluent verbal and written English and Spanish communication preferred; Knowledge of specific job-related systems (i.e., BALC, BQM, IDPH, & IDHS rules and regulations); Completion of QIDP training certification. EMPLOYMENT TYPE: Full-Time Hourly WAGE/SALARY RANGE: $26.00 – $28.00 / hour (based on experience) WORK LOCATION: 1850 W 21st Street, Chicago, IL 60608 – Flexibility to travel to satellite locations required.
