Job Description - Clinical Pharmacist Specialist
Clinical Pharmacist Specialist (24045771) is a full-time position with a schedule of 10am-6:30pm, weekdays, and rotating weekends (every 3rd weekend) at 301 Memorial Medical Parkway, Daytona Beach, FL 32117.

Role:
The primary responsibility is to provide integrated clinical and distributive pharmacy services, including decentralized interprofessional pharmacy practice, direct patient care programs, and leadership of quality improvement initiatives.
Responsibilities:
Provide general and specialty direct patient care programs, advanced drug management services, and medication use systems.
Actively participate in medication safety systems, regulatory compliance initiatives, patient quality services, and disease outcomes programs.
Deliver medication therapy management consult services and disease state management services as needed.
Participate in outstanding customer service and maintain respectful relationships.
Pharmacy Operations:
Perform all pharmacist functions, participate in standard work parameters, and maintain functional proficiency in all pharmacy areas. Oversee medication automation, technology, and computer systems, and manage medication inventories.
Pharmacy Practice:
Complete critical medication order scrutiny, drug profile review, and patient monitoring to identify, prevent, or mitigate medication-related problems. Provide patient-centered care services, including accurate, safe, and timely medication therapy.
Clinical Pharmacy Services:
Provide primary coverage of direct patient care programs and provide accurate, safe, timely, and appropriate medication therapy based on patient needs. Collaborate with healthcare providers and pharmacy staff to deliver patient-centered care.
Qualifications:
The expertise and experiences you’ll need to succeed:
Bachelor's degree in Pharmacy
Board of Pharmaceutical Specialties Certification
Licensed Pharmacist
Post Graduate Year One (PGY1) AND Post Graduate Year Two (PGY2) Pharmacy Residency in specialty area consistent with practice needs of institution
OR
PGY1 Residency plus contemporary hospital or specialty practice experience (3+ years) consistent with practice needs of institution
OR
Clinical pharmacy experience in contemporary hospital or specialty practice (5+ years) consistent with practice needs of institution
