Hanford Post Acute , where we provide high-quality, person-centered care in a skilled nursing setting. We are currently seeking a
Speech Language Pathologist (SLP)
to evaluate, plan, and implement therapy services to meet the communication and swallowing needs of our residents. Responsibilities Include: Evaluate patients within 24 hours or 1 business day of physician referral (with documentation if delayed)
Develop and implement personalized treatment plans approved by referring physician
Provide direct treatment and document patient progress in compliance with state and facility standards
Supervise SLP Assistants and Clinical Fellows as needed
Participate in interdisciplinary meetings, care conferences, and in-service training
Provide caregiver education and discharge planning
Maintain accurate and timely documentation in systems like Casamba and PointClickCare
Comply with all applicable laws, regulations, and facility policies
Qualifications: Bachelor’s degree in Speech-Language Pathology required; Master’s or Doctorate preferred
Active and unencumbered California SLP license
CPR certification and continuing education per state requirements
Strong clinical reasoning and interpersonal communication skills
Experience in SNF setting preferred
Proficient in electronic documentation systems (e.g., Casamba, PCC)
Physical Demands & Work Environment: Frequent standing, walking, reaching, and lifting up to 50 lbs
Occasional stooping, kneeling, or crouching
Moderate noise level typical of a healthcare facility
