The Clinical Pharmacist supports the mission of the department, hospital, and health-system by preventing, identifying and resolving medication use issues to assure patient safety, improve patient satisfaction, optimize outcomes and minimize cost. May work in centralized and decentralized areas. Will perform all functions of a registered pharmacist including accurate and timely dispensing, clinical interactions with patients and the healthcare team, and supervision of technicians. We are seeking a dedicated and experienced Ambulatory Clinical Pharmacist to join our team, specializing in management of specialty medications across neurology, rheumatology, gastroenterology, and cardiology. Responsibilities include managing and overseeing the program for specialty medications to ensure optimal patient outcomes, providing comprehensive medication reviews, and developing personalized therapy plans for patients with complex conditions. The pharmacist will collaborate with physicians, nurses, and other pharmacists to ensure coordinated patient care. They will educate patients, monitor progress and adherence, and execute therapy plans as needed. Staying current with developments in specialty medications and ensuring regulatory compliance are essential responsibilities. Pharmacy Operations — participates in pharmacist functions, coverage of central/satellite areas, oversees medication automation and inventory, and supervises technicians. Pharmacy Practice — medication order scrutiny, patient monitoring, drug interactions, patient-centered care, cost-effective therapy, stewardship and safety programs, and emergency response. Clinical Pharmacy Services — direct patient care programs, collaboration with healthcare providers, and patient-centered care delivery. Teamwork and Inter-professional Collaboration — continuity of care across shifts and practice areas, effective teamwork. Teaching, Training, Precepting, Mentoring — mentoring staff and training trainees; supports education of students and nurses as requested. Quality Improvement, Regulatory Compliance, and Accreditation — supports improvement initiatives and compliance programs. Qualifications The expertise and experiences you'll need to succeed: EDUCATION AND EXPERIENCE REQUIRED: Bachelor of Pharmacy, Doctor of Pharmacy, or other advanced pharmacy degree EDUCATION AND EXPERIENCE PREFERRED: PGY1/PGY2 (Ambulatory Care) Pharmacy Residency

OR

Three years of clinical pharmacy experience LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED: Applicable State Pharmacy License BLS LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED: BLS, ACLS, PALS and/or NRP may be required for some facilities based on department requirements and pharmacist role, as defined by specialty unit.
