Overview
The Wound/Skin Care Coordinator is responsible for coordinating, implementing, and monitoring a facility approved skin care program, which includes prevention and treatment of skin problems related to impaired skin integrity. Responsibilities
Represent Independence Care Center in a positive and professional manner. Know and practice Resident’s rights. Demonstrate a caring attitude in all interactions with residents. Abide by Independence Care Center Personnel Policies and Procedures as outlined in the Employee Handbook. Know and practice emergency policies and procedures. Provide wound and skin care to all residents as ordered by their physician. Must treat residents with respect and allow them to maintain their dignity. Do a weekly assessment of all residents with ulcers. Call family or DPOA weekly with update. Complete and submit a weekly report to DON for review. Complete Pressure Ulcer Risk Assessment on all new admissions. Reassess quarterly thereafter. Record Risk Assessment scores on flow sheet. Update as needed. Assess residents identified at risk, at least bi-weekly, with documentation on flow sheet. Provide pressure reduction devices for those residents identified at risk for development of pressure ulcers. Update Wound and Skin Care Protocols as needed. Submit to DON for review and approval. Monitor program for compliance. Participate in Care Plan meetings, as needed. Work as LPN charge nurse if scheduled to do so. Participate in QI Committee Meeting. Responsible for assuring Resident safety. All other duties as assigned. Qualifications
QUALIFICATIONS: Must be a graduate of a RN or LPN training program and have current RN or LPN licensure in the State of Missouri. Must be at least 18 years of age. Must be able to read, write, and speak fluently in English. Must be able to communicate with residents and families in a pleasant and supportive manner. Also, must be able to work with and communicate effectively with staff. PHYSICAL AND SENSORY REQUIREMENTS
Must be able to stand, bend, reach, kneel, carry, lift 75-100 lbs., and move intermittently during working hours. Must be willing and able to work a flexible work schedule when necessary. To assist residents in a time of emergency must be able to move quickly from one part of the building to another. Must have good physical health with no communicable diseases present. Must be able to demonstrate emotional stability in order to cope with the mental and emotional stress of the position. Must possess manual dexterity for operating a computer and other like equipment. Exposed to body fluids, infection, odors, and behavior of residents.
