Nurse Practitioner / Physician Assistant – Neuro‑Hospitalist Location: HMHMG Specialty Care, 570 Edison, New Jersey

Overview The Neuro Hospitalist Advanced Practice Nurse (APN) or Physician Assistant utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN/PA works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs. Shift expectation M‑F, weekend rotation as needed with the current team.

Responsibilities

Perform and document accurate, complete, and relevant history and physical assessments appropriate for the patient’s age, gender, and clinical problem.

Order, perform, and interpret laboratory tests and diagnostic studies/procedures in collaboration with physicians and consultants as appropriate.

Manage general medical and surgical conditions based on comprehensive knowledge of etiologies, risk factors, pathophysiology, and the indications and contraindications of pharmacologic agents and other treatment modalities.

Prescribe medication safely and effectively in accordance with best practices, hospital policy, and NJ regulations, providing patient education as needed.

Seek opportunities to promote HMH APN/PA practice recognition through conferences, publications, and presentations.

Provide patient‑ and family‑centered, compassionate, and effective care promoting health and treatment of disease.

Practice as a healthcare provider according to the rules and regulations of the Medical Staff and bylaws as outlined in the delineation of privileges.

Adhere to HMH nursing protocols, standards of care, and disease‑specific quality indicators.

Apply evidence‑based and value‑based medicine to systematically analyze and improve patient care practices and outcomes.

Evaluate patient/family transition of care planning, considering resource utilization, cost, quality, safety, and self‑management promotion for chronic conditions.

Perform medication reconciliations and maintain accurate medication lists.

Participate in annual APN/PA peer review and clinical quality improvement activities.

Facilitate consistent, coordinated care and clear communication among all members of the healthcare team through daily interdisciplinary rounds.

Advocate for patients and families, identifying and addressing obstacles to care progression and escalating issues as appropriate.

Serve as a clinical resource for the multidisciplinary team, addressing clinical, process, and system improvement opportunities.

Collaborate with medical staff to support nursing‑led protocols and mentorship of other APNs/PA’s.

Participate in organizational and departmental meetings, committees, and workgroups as necessary.

Demonstrate the highest level of accountability for professional practice and adhere to ethical principles regarding patient confidentiality, informed consent, and adverse outcomes.

Maintain adherence to legal framework and scope of practice defined by NJ Division of Consumer Affairs and HMH organizational competencies.

Perform other duties and/or projects as assigned.

Qualifications – Nurse Practitioner

Graduate of an NLN/AACN accredited nursing program.

Master’s or Doctorate degree from an accredited Nurse Practitioner program.

Maintains clinical competencies through continuing education and clinical practice.

Experience in a relevant patient care setting.

National Provider Identifier (NPI).

Prescriptive authority through CDS and DEA registration according to New Jersey state law.

Collaborative agreement with physicians on staff.

Excellent written and verbal communication skills.

Proficient computer skills including Microsoft Office and/or Google Suite platforms.

Desired Licenses & Certifications – Nurse Practitioner

NJ State Professional Registered Nurse License.

Advanced Nurse Practitioner (w/ Master’s Degree).

BLS AHA Basic Health Care Life Support HCP Certification.

Board certified by AANP or ANCC.

Advanced Cardiac Life Support Certification (preferred).

Qualifications – Physician Assistant

Successful completion of an approved physician’s assistant program, accredited by AMA Council on Medical Education.

Excellent written and verbal communication skills.

Proficient computer skills including Microsoft Office and/or Google Suite platforms.

Desired Licenses & Certifications – Physician Assistant

Physician Assistant License.

National Commission on Certification of Physician Assistants.

BLS AHA Basic Health Care Life Support HCP Certification.

Advanced Cardiac Life Support Certification (preferred).

Compensation Minimum rate of $148,803.20 Annually.
