$75,545.60/yr - $125,923.20/yr All nursing practice is based on the legal scope of practice, national and specialty nursing standards, CNMC Policies and Procedures, and in accordance with all applicable laws and regulations. The Professional Model of Care requires registered professional nurses to be responsible and accountable for their own practice. CNMC supports the development of RN relationships within the community; specifically, those relationships associated with the health and wellbeing of the community at large. The Virtual Care Coordinator

supports operations of the BOV Telehealth Command Center at Children’s National Hospital. Provides virtual surveillance for patients with critical congenital or acquired heart disease in the CICU at Children’s National Hospital, as well as others in remote institutions nationally or internationally. Monitors patient vital signs and trends from multiple monitoring devices, and alerts bedside teams of concerning trends when appropriate. Coordinates nursing care and services for patients and family members in specific healthcare specialty encounters by telephonic, electronic, in person, and/or by mail. Consults with physicians and other healthcare providers to resolve health concerns. Incorporates all members of the health care team in the planning and delivery of patient care. Handles complex clinical problems and conducts quick assessments of patients through the command center, managing emergency situations appropriately. Demonstrates evidence-based practice through participation in unit projects and activities. Responsible for leading the activities of the patient care delivery team for specific patient populations and ensuring continuity of care. Minimum Education Bachelor’s Degree in Nursing (Required) Minimum Work Experience 5 years nursing experience with a minimum of two (2) years’ recent pediatric experience in a comparable setting and a minimum of one (1) year of demonstrated nursing leadership experience (e.g., charge nurse, educator, preceptor, council chair/member or comparable roles) (Required) Required Skills/Knowledge Prior telehealth experience (Preferred). Successfully completes the hospital and nursing orientation at CNMC. Knowledge of pediatric healthcare standards with related clinical skill competency. Strong knowledge of JC and other regulatory agency requirements. Demonstrated leadership and management skills. Excellent written and verbal communication skills. Proven capacity to make critical clinical decisions when appropriate. Required Licenses And Certifications Registered Nurse (Required) or RN licensure in the state of practice or supervision (Required) Functional Accountabilities Professional Practice/Research Participate in/support Shared Nursing Leadership. Contribute to the sustainability and advancement of nursing excellence which translates into ongoing Magnet designation. Participate in internal/external committees, professional organizations, and community activities. Support/participate in the Division of Nursing’s performance improvement process. Identify, define, and report on measures consistent with Nurse Sensitive Indicators (NSIs) for Ambulatory Nursing Practice. Integrate evidence-based practice and research into practice. Assist physicians and other professional providers to maintain appropriate cost and desired patient outcomes. Patient Family-Centered Care Plan to consult and collaborate with patients, families, physicians, nurse practitioners, physician assistants and other professional colleagues. Involve patients and families in shared decision making; advocate for patient and family; accessing relevant health services. Exercise authority and accountability for clinical care for specific patient populations and area workflow. Develop pertinent goals based on patient response to illness, health promotion and program expected outcomes; implement suitable, evidence-based nursing interventions; provide careful and complete documentation of care provided. Use the nursing process in the clinic setting, at the bedside, and in telehealth services to assess patient problems and concerns; critically analyze and integrate subjective and objective data. Provide health education based on patient/family learning preferences throughout a patient’s disease trajectory or health promotion; utilize cultural competency to enhance relationships, processes, and outcomes for patients/families. Engage opportunities to act in the best interest of the patient/family, building trust and confidence, while upholding moral and legal standards of due care. Coordinate mechanisms across systems, institutions, and community to provide continuity of care for wellness needs or health concerns. Direct and participate in the development of patient care policies and protocols to advise and guide practice. Resources and Work Environment Establish and maintain medical equipment and supplies; promote safe and effective use of equipment. Ensure regulatory requirements are met and area is in a constant state of readiness in the area of practice; stay updated on health informatics systems linked to patient care. Apply knowledge of operations, financial principles, and resources to ensure quality, safe, and cost-effective care services. Enhance knowledge, skills, and abilities in clinical, legal, regulatory, and cultural competence related to the patient population and community care. Foster collaboration to create, promote, and maintain an environment that supports high-performance teams. Partners for Excellence Facilitate interdisciplinary patient care rounds/conferences to review treatment goals and optimize outcomes. Ensure time, schedule, and alternate coverage accuracy to meet patients’ and families’ care and personal needs; participate in cross-coverage of other areas when needed. Direct the flow of patients and staff to ensure patients are adequately managed and the care environment runs smoothly. Collaborate with professional staff including RNs, LPNs, PCTs and other unlicensed assistive personnel as defined by state practice acts and CNH job descriptions. Communicate and escalate clinical/administrative issues as appropriate to designated resolution points. Participate in peer review for staff. Stop in the face of uncertainty and take time to resolve the situation. Demonstrate accurate, clear, and timely verbal and written communication. Job Specific Responsibilities Respond to routine, urgent, and emergent patient issues at CNH Critical Care bedside; provide follow-up to ensure problem resolution. Collect and analyze patient data from remote monitoring and multiple data sources from patients in the CICU at Children’s National Hospital and others in remote institutions as directed. Communicate with Command Center and bedside CICU teams to provide safe, error-free care during routine and emergent conditions. Document patient physiologic health status and concerning trends as appropriate. Demonstrate knowledge of the program’s top 10 triage/symptom guidelines and use decision support tools for evidence-based practices. Collaborate with bedside teams to solve clinical problems using virtual monitoring and AI tools; report unresolved issues to medical staff or CICU nurses as appropriate. Apply AI features in the Command Center to enhance assessment and documentation during virtual surveillance. Participate in expanding the command center care model to other inpatient, ambulatory, and emergency room care models and enhance AI tools. Achieve telehealth and other applicable national certifications.
