Summary The Psychiatric Advanced Practice Provider delivers psychiatric care to ICE detainees, including evaluations, treatment plans, medication management, and crisis intervention. The role involves collaboration with psychiatrists, clinical directors, and consultants, mentoring peers, and supporting policy development. It requires psychiatric certification, clinical expertise, and adaptability to schedule changes.

Duties and Responsibilities

Provides direct psychiatric care to detainees via onsite and telepsychiatry services for scheduled appointments, emergency, or urgent care visits.

Consults with the Psychiatrist and/or Clinical Director for complex psychiatric patients and as required by IHSC policy.

Provides infirmary care including admission, discharge, and daily rounds for detainees requiring higher levels of psychiatric care or monitoring.

Performs comprehensive psychiatric evaluations and initiates treatment plans as required by IHSC policy.

Works within IHSC Psychiatric Advanced Practice Provider Collaborative Practice and Prescriptive Authority Agreement and the IHSC formulary.

Follows IHSC policies in managing significant mental illness—including prevention of violence, homicide, self‑harm, and suicide.

Applies knowledge of drugs and therapeutics to prescribing medication.

Applies patient‑specific knowledge to monitor and control stress levels and provide emotional support and counseling to detainees.

Coordinates psychiatric care needs of detainees, including referrals to off‑site consultants.

Solicits and reviews pertinent psychiatric records, interprets diagnostic and laboratory results as required.

Adheres to universal precautions and may serve as a medical escort for domestic/overseas travel.

Utilizes and documents interpretation or assisted communication services for detainees with Limited English Proficient or communication disabilities.

Maintains flexibility to sudden changes in schedules and works to provide on‑call coverage as needed.

Provides relevant patient education to detainees verbally or with handouts.

Documents detainee care visits in the required health record in compliance with IHSC policy.

Maintains patient confidentiality and records confidentiality.

Completes all required training and may serve in an acting capacity as part of the IHSC Psychiatry Services Program leadership team.

Mentors Psychiatric APP onboarding and training, attends regional and national meetings, participates in peer review, and assists in program development.

Travel may be required up to 20% annually.

Performs other duties as assigned.

Requirements

U.S. citizenship.

Successful background investigation and drug screen.

Male applicants born after 12/31/59 must certify Selective Service registration.

Meet all requirements by the closing date.

Upon appointment, officers will serve an 18‑24 month commitment to IHSC.

Applicants must be a current USPHS Commissioned Corps Officer or USPHS Call to Active Duty (CAD) candidate; civilians are not eligible.

Must not have a current Assignment Pay Agreement; if one exists, it must be fulfilled within 90 days of application.

PHS officers must participate in IHSC TDY On‑Call Schedule (ITOS).

Qualifications

Six months of full‑time experience as a Psychiatric APP in direct patient care or expressed approval of the IHSC Chief Psychiatrist.

PMHNP licensure: unrestricted state license and national board certification.

PA licensure: unrestricted state license and national board certification.

Doctoral or master’s degree from an accredited nursing or physician‑assistant program.

DEA registration for prescribing controlled medications within 90 days of start date.

Maintained CPR/BLS certification and continuity of licensure.

Comprehensive knowledge of managing non‑compliant patients and special needs populations.

Physical Demands

Walk unaided at a normal pace up to 5 minutes; jog or fast walk up to ¼ mile.

Perform CPR or emergency care standing or kneeling.

Assist in moving or carrying up to 30 pounds.

Work in a stressful and austere environment without physical limitations.

Preferred Knowledge, Skills, and Abilities

Knowledge of forensic psychiatry concepts and principles.

Correctional health care experience.

Certification as a Correctional Healthcare Professional (CCHP).

Experience with IHSC policies, NCCHC, and PBNDS requirements.

Bilingual communication capability preferred.

Additional Information This position is located within DHS, ICE, ERO, ICE Health Service Corps, Office of the Deputy Assistant Director of Clinical Services, Psychiatric Services Program. The incumbent will serve as a Psychiatric Advanced Practice Provider, reporting to the facility clinical director and establishing a collaborative practice agreement with a psychiatrist.

Applicants must be eligible to work in ICE detention facilities nationwide, providing direct patient care at 19 primary facilities and facilitating off‑site medical support at approximately 240 IGSA facilities.
