Job Overview The Care Manager, also called Designated Care Manager, is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long‑term care and reminiscence neighborhoods. As the Care Manager, you will provide hands‑on care and physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP), while maintaining a safe and comfortable home‑like environment.

Job Description Responsibilities & Qualifications Essential Duties As part of the Sunrise team, supporting our Mission, Principles of Service and Core Values is a fundamental part of this job.

Review, read, notate, and initial daily logs to document pertinent resident information.

Participate in the development of ISPs and monthly updates.

Review designated assignments and maintain familiarity with resident locations throughout shifts.

Observe, report, and document resident symptoms and conditions, including changes in skin, behavior, alertness, weight, diet, and activity participation.

Attend daily Cross‑Over meetings with the lead care manager.

Notify supervisors and resident care directors when a resident’s care needs increase.

Respond promptly to security system and resident call bells, including resetting call bells when necessary.

Greet guests, family members, residents, and team members warmly.

Answer, direct, and respond to phone calls in a timely, courteous, and professional manner.

Assist with continence management and dispose of all continence products properly.

Communicate with families as needed and serve as a resource for resident support.

Help residents maintain independence, physical safety, and dignity, adhering to Resident Rights and Sunrise Principles of Service.

Participate in or lead residents with activities of daily living (ADLs) and invite, encourage, and assist (IEA) residents to participate in activities per ISP.

Engage residents in life‑skills and reminiscence enrichment activities throughout the day.

Strive to understand and respond to each resident with empathy and respect for their unique communication patterns and needs.

Maintain a warm, personal, and positive atmosphere while ensuring safety regulations are always followed.

Practice routine care of cleanliness, hygiene, and health.

Host and engage in daily resident activities.

Risk Management and General Safety

Partner with the community team to ensure compliance with national and provincial safety requirements.

Practice safety procedures, including PPE, fire extinguishers, SDS, and lockout‑tagout.

Report all accidents, incidents, unsafe, and hazardous conditions immediately.

Ensure cords, carts, and equipment are kept clear of exits and in compliance with fire codes.

Handle exposure to blood‑borne pathogens, bodily fluids, contaminated waste, sharp objects, and hazardous materials responsibly.

Maintain safe storage and monitoring of oxygen tanks.

Housekeeping and Laundry Services

Maintain and clean resident rooms, including making beds, emptying trash, putting clothes away, spot sweeping, and mopping as needed.

Wash resident laundry as noted in ISP.

Wash and fold dining room linens and napkins.

Complete assigned housekeeping tasks and maintain common area cleanliness.

Dining Service

Serve meals and assist with dining room setup and cleanup.

Promote a pleasant dining experience during all meals.

Participate in pre‑meal meetings and follow resident diet plans per ISP.

Observe and document changes in resident dining habits.

Provide room service delivery as needed.

Practice safe food handling in compliance with universal care precautions.

Specific Responsibilities for the Reminiscence Neighborhood Care Manager

Invite residents in life‑skills and other enrichment activities as per ISP.

Encourage residents to attend afternoon socials.

Integrate the concept of 'Creating Pleasant Days' into daily routines.

Blend multi‑sensory experiences into resident care.

Participate in monthly letters home and letter‑writing with residents and families.

Training and Team Success

Participate as a team member committed to team goals.

Demonstrate the Team Member Credo in daily interactions.

Serve residents and guests through Principles of Service.

Contribute to engagement programs, including training and survey participation.

Attend regular meetings and training sessions as directed.

Maintain compliance with infection control and bio‑hazard procedures.

Complete required training per state regulations and Sunrise standards.

Perform other duties as assigned.

Experience and Qualifications

High School diploma or GED accepted and may be required.

CPR and First Aid certification as required by state regulations.

Must be at least 18 years old.

Previous experience working with seniors preferred.

Desire to serve and care for seniors.

Ability to make choices and decisions in the resident’s best interest.

Drivers must sign the Driver job description and understand safety compliance.
