Overview
Care Manager/“Designated Care Manager” is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands on care and physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP) while maintaining a safe and comfortable home like environment. The Care Manager demonstrates the Mission for Sunrise Senior Living, “to champion quality of life for all seniors” in accordance with federal, state/provincial, and local laws and regulations and Sunrise Senior Living standards, policies, and procedures to promote the highest degree of quality care and services to our residents. Base pay range

Essential Duties Support our Mission, Principles of Service and Core Values; document and learn about residents through Daily Log notes and ISPs Participate in the development of ISPs and monthly updates; review designated assignments; be responsible for a designated group of residents during the shift and physically check on them Observe, report, and document symptoms and conditions (e.g., skin, behavior, alertness, weight, diet, activity participation) Attend daily Cross Over meetings by the lead care manager; notify supervisor and resident care director if a resident has increased care needs Inform supervisor of any resident changes in condition; respond to call bells and ensure quick action Greet guests, family members, residents, and team members; answer calls professionally Assist with continence management and dispose of products per sanitation standards; communicate with families as needed Help residents maintain independence and safety; adhere to Resident Rights and Sunrise Principles of Service Assist with ADLs and encourage participation per ISP; engage residents in life skills and reminiscence activities Maintain warmth and a calm environment; follow safety regulations; practice Standard Care Precautions Host and engage in daily activities with residents Risk Management and General Safety Ensure community compliance with occupational health and safety regulations; promote Risk Management programs Follow safety procedures including PPE, fire safety, SDS, Lockout/Tagout; report accidents/incidents and hazards Manage hazards and ensure safe storage of oxygen and other materials Housekeeping and Laundry Maintain and clean resident rooms; handle laundry and linens; keep common areas tidy Dining Service Serve meals, promote a pleasant dining experience, assist with setup/cleanup, participate in pre-meal meetings; follow ISP diet guidelines; observe changes in dining habits; provide room service as needed; practice safe food handling Reminiscence Neighborhood IEA residents in life skills and activities; integrate life skills into daily routine; create pleasant days with multi-sensory experiences; monthly letters home with residents and families Training and Team Participate as a team member; contribute to engagement programs; attend meetings and trainings; comply with infection control and regulatory training Core Competencies Communication, multi-tasking, judgment, problem solving and time management Qualifications
High School diploma/GED; CPR and First Aid as required by regulations At least 18 years old; previous experience with seniors preferred Desire to serve and care for seniors; ability to act in resident’s best interest If applicable, drivers must sign Driver job description for safety and regulatory compliance Pre-employment and Benefits
