As a Nurse Navigator, you will serve as an advocate and educator for patients with all types of medical conditions, supporting them through complex healthcare environments to optimize care and outcomes. You will facilitate follow‑up through coaching, information transfer, and communication to improve continuity of care, streamline the intervention process, and assist families with access issues. Your role includes building relationships with patients and physicians, coordinating care plans, scheduling appointments, resolving barriers, providing education, and connecting with community resources. You will also manage program resources efficiently, collaborate across facilities, and collect data to track navigation program performance. Responsibilities

Act as a patient advocate. Assess medical and psychosocial needs for navigation. Use appropriate tools to identify patient needs and barriers, and provide access to potential resolutions. Identify health disparities and assist in their removal. Provide education, outreach, and support to patients and families, ensuring empowerment in managing healthcare needs. Guide patients through the healthcare system and promote adherence to recommended treatment. Assist providers in understanding diagnoses, treatment options, and available resources, including clinical research studies and technological advances. Connect patients to community and external resources such as supportive care, financial support, and return‑to‑work programs. Streamline appointment scheduling in a compliant manner and explain treatment sequences. Ensure smooth transitions between care modalities, facilities, and providers. Collaborate with physicians to ensure patient compliance and follow‑up communication of concerns or changes. Participate in multidisciplinary meetings and other care planning conferences as required. Maintain accurate patient data, including assessment and documentation. Educate patients and families about planned procedures, medications, and treatments. Qualifications

1–3 years of experience in Nursing – required. 3+ years as a Nurse Navigator and/or patient educator in a service line – preferred. Education

Associate’s Degree in Nursing – required. Bachelor’s Degree in Nursing – preferred. Licenses and Certifications

Basic Cardiac Life Support (preferred if remote). Registered Nurse – currently licensed in the state where the employee practices. Equal Opportunity
