Lung Nodule Nurse Navigator

Job Summary and Qualifications
Act as a member of the multidisciplinary team and advocate for patients, facilitating follow‑up and communication to improve continuity of care.
Guide patients and families through complex healthcare environments, assist with access issues, appointments, and resolution of barriers.
Build relationships with patients and physicians, coordinate defined care plans, and provide education and community resource connections.
Deliver timely data collection and program performance tracking aligned with organizational goals.
Conduct follow‑up conversations, attend multidisciplinary meetings, and ensure appropriate patient data is documented.
Requirements
1–3 years of nursing experience (required).
3+ years as a nurse navigator and/or patient educator in a service line (preferred).
Associate’s degree in nursing (required); Bachelor’s preferred.
Basic Cardiac Life Support (preferred for remote role).
Registered Nurse license in the state of practice.
