Responsibilities Manage patient care plans and coordinate services for optimal outcomes.
Conduct assessments to identify patient needs and develop care strategies.
Collaborate with healthcare teams to ensure comprehensive patient support.
Monitor patient progress and adjust care plans as necessary.
Educate patients and families on care plans and health management.
Utilize data to track performance metrics and improve service delivery.
Participate in professional development opportunities to enhance skills.
Ensure compliance with healthcare regulations and standards.
Maintain accurate documentation of patient interactions and care plans.
Engage in continuous learning to stay updated on best practices.
Develop and implement strategies to achieve performance targets.
Provide support and guidance to junior staff and new team members.
Experience/Qualifications Registered Nurse (RN) license in Maryland or compact state required.
Commitment to continuous learning and professional development.
Experience in case management and patient care coordination is essential.
Proven track record of meeting performance targets and goals.
Strong communication skills to educate patients and collaborate with teams.
Knowledge of healthcare regulations and compliance standards.
Ability to analyze data for performance metrics and quality improvement.
Ability to adapt care plans based on ongoing patient assessments.
Familiarity with electronic health records and documentation practices.
Experience mentoring or supporting junior staff is a plus.
