LPN Med Care Manager

Job Overview The LPN Medication Care Manager is responsible for providing the highest degree of quality care by administering medications and treatments in a safe, organized manner. Responsibilities include medication administration, documentation of medication administration, conducting SHUs, and providing resident care consistent with Sunrise’s Mission: to champion the quality of life for all seniors.

Medication Administration
Provide the highest degree of quality care by administering medications and treatments safely and in an organized manner.
Review, read, notate, and initial the electronic health record to document pertinent information about residents.
Receive medication updates from the Resident Care Director (RCD) or Wellness Nurse.
Administer, assist with, and observe medications and treatments using the medication administration record and the Six Rights of Medication Pass.
Ensure medications are passed according to schedule using a mobile medication cart.
Document and initial medications as given; document refusals or missed doses appropriately.
Maintain confidentiality of all resident information including medications.
Report resident concerns and unavailable medications to the RCD or Wellness Nurse.
Restock medication cart after passes; assist in checking medications regardless of packaging.
Assess residents for as-needed medications and document/report appropriately.
Count all narcotics with another MCM or Lead Care Manager and report discrepancies.
Maintain and clean the Medication Room, carts, and treatment carts; manage medication stock and expiry.
Follow the refill process and promote resident independence and safety per Sunrise Principles of Service.
Ensure proper infection control, hygiene, and safety practices; understand exposure protocols for bloodborne pathogens.
Audit medication carts as part of daily responsibilities.
Resident Care
Notify RCD of resident and/or family concerns.
Participate in daily Cross Over meetings.
Conduct Service and Health Updates as directed by the RCD.
Participate in the development of Individualized Service Plans (ISP).
Transcribe orders as required.
Risk Management and General Safety
Ensure compliance with national/provincial regulations and promote risk management programs.
Practice safety procedures including PPE, fire safety, SDS, and Lockout/Tagout.
Report accidents/incidents and unsafe conditions immediately.
Keep cords, carts, equipment, and hazards arranged to comply with fire codes.
Follow infection control techniques and proper disposal procedures for hazardous materials.
Understand and practice safety for exposure to blood, bodily fluids, infectious materials, and hazardous chemicals.
Ensure safe storage of oxygen tanks and monitor prescribed levels.
Training and Team Contribution
Be a member of the team and contribute to team goals; demonstrate Sunrise Credo in daily interactions.
Serve residents through the Principles of Service; engage in development and engagement initiatives.
Attend regular meetings and training; may be designated as shift supervisor or supervise other MCMs.
Maintain compliance with required trainings and regulatory standards.
Perform other duties as assigned.

Make decisions in the resident's best interest; stay calm in difficult situations.
Handle multiple priorities; communicate effectively; demonstrate sound judgment and problem-solving.
Experience and Qualifications To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed are representative of the knowledge, skills, and abilities required.
LPN, LVN, or state/province-specific licensed nurse credential.
Maintain applicable certifications where required by state/provincial regulations.
CPR and First Aid certifications.
Must be at least 18 years of age.
Previous experience working with seniors preferred.
Desire to serve and care for seniors.
If applicable, comply with Sunrise driver requirements and safety duties.
