Sunrise Senior Living is hiring a Licensed Practical Nurse (LPN) Case Management to provide high-quality care and medication administration to residents in a safe, organized manner. This role supports the Sunrise Mission: to champion the quality of life for all seniors, in accordance with laws, standards, and Sunrise policies.
Job Overview The LPN Medication Care Manager administers medications and treatments, documents administration, conducts SHUs, and provides resident care aligned with Sunrise core values and policies.
Job Description Responsibilities & Qualifications
Essential Duties Responsibilities
Support the Sunrise Mission, Principles of Service and Core Values; uphold the sacred value of human life.
Medication Administration
Provide quality care by administering medications and treatments safely and in an organized manner.
Review and document information in the electronic health record.
Receive medication updates from the Resident Care Director (RCD) or Wellness Nurse.
Administer, assist with, and observe medications and treatments using the Six Rights of Medication Pass.
Ensure medications are passed on time using a mobile medication cart.
Document administration, including refusals or missed doses.
Maintain confidentiality of resident information pertaining to medications.
Report concerns and unavailable medications to the RCD or Wellness Nurse.
Restock medication carts after passes; assist in checking medications regardless of packaging.
Assess residents for as-needed medications and document appropriately.
Count narcotics with another MCM or LCM each shift and report discrepancies.
Maintain and clean Medication Room, carts, and treatment carts; follow refill processes.
Promote resident independence, dignity, and safety in line with Sunrise Principles of Service.
Demonstrate empathy, considering each resident’s communication patterns and needs.
Practice standard care precautions for cleanliness and hygiene; audit medication carts.
Resident Care
Notify RCD of resident and/or family concerns.
Participate in daily Cross Over meetings and conduct Service and Health Updates as directed by RCD.
Contribute to Individualized Service Plans (ISP) development and transcribe orders as needed.
Risk Management & Safety
Ensure compliance with national/provincial occupational health and safety regulations; promote Risk Management programs.
Follow safety procedures including PPE, fire safety, SDS, and Lockout Tagout as applicable.
Report accidents/incidents and unsafe conditions immediately.
Keep cords, carts, equipment, and hazards out of walkways and in compliance with fire codes.
Follow infection control procedures and proper disposal of hazardous materials.
Handle exposure to bloodborne pathogens and hazardous materials per policy; store oxygen tanks safely.
Training & Team Success
Be a collaborative team member; contribute to engagement and performance improvement efforts.
Attend regular meetings and training; may serve as shift supervisor or supervise other MCMs as assigned.
Maintain required trainings in line with Sunrise and regulatory authorities.
Perform other duties as assigned.
Core Competencies
Make decisions in the resident's best interest; stay calm in difficult situations.
Handle multiple priorities; strong written and verbal communication; good judgment and problem-solving.
Experience & Qualifications
LPN, LVN, or state/province licensed nursing credential.
Maintained certifications where applicable; CPR and First Aid.
18 years of age or older; previous experience with seniors preferred.
Desire to serve and care for seniors; driving role may require a Driver job description where applicable.
