Job Description What you will be doing:
Conduct physical exams and diagnostic assessments for adult patients with acute and chronic illnesses
Develop treatment plans in alignment with clinical protocols and national standards
Provide health education on disease prevention, nutrition, and lifestyle
Administer or prescribe medications per standing orders or in collaboration with supervising physician
Coordinate specialty referrals, chronic care plans, and follow-up care
Serve vulnerable populations, including aging and homeless adults
Document patient care thoroughly and accurately in the EMR
Participate in clinical quality improvement, peer review, and case conferences
Collaborate with behavioral health, nursing, and care coordination staff to ensure integrated care
Rotate in after-hours call pool, work one late shift (10 AM–7 PM) per week, and one Saturday half-day every 6–7 weeks
Experience you will need
Current licensure in Hawaii as an APRN, FNP, MD, or DO
Active CPR certification and negative TB test (within past 12 months)
1+ year of clinical experience post-licensure in a primary care or community setting
Demonstrated commitment to serving diverse populations with empathy and cultural competence
Strong clinical documentation and team communication skills
Our client asked me to submit 3 great people within the next few days. We work directly with the hiring manager and can arrange interviews within a few days.
