The Family Physician Assistant plays a crucial role in delivering comprehensive healthcare services to patients of all ages within the Long Island Select Healthcare Inc. facilities. This position involves collaborating closely with physicians to assess, diagnose, and treat a variety of medical conditions, ensuring that patients receive high-quality care tailored to their individual needs. The Family Physician Assistant will also be responsible for educating patients about their health, preventive care, and managing chronic conditions, thereby promoting overall wellness in the community. By maintaining accurate medical records and following up on patient progress, this role contributes significantly to the continuity of care. Ultimately, the Family Physician Assistant is dedicated to enhancing patient outcomes and fostering a supportive healthcare environment. Minimum Qualifications
Bachelor's degree in Physician Assistant studies or a related field. Current certification as a Physician Assistant (PA-C) and valid state licensure. Preferred Qualifications
Experience in family medicine or primary care settings. Bilingual proficiency in English and Spanish or other languages. Responsibilities
Assessment and diagnosis of patients presenting with various clinical problems and complaints. Provide information about the Patient Centered Medical Home (PCMH) and Chronic Care Management (CCM) programs to patients as relevant to their care within LISH. Obtain a waiver for Medically Assisted Treatment if not already on file, within 60 days of hire, with time allotted during work hours to accomplish this. Become proficient in telemedicine and provide care via this platform as required. Assist in the educational growth of the agency by allowing students to shadow as needed. Be available for committees such as Quality Assurance, peer review and root cause analysis to provide input into workflow and improvements. Devise, implement and evaluate plans of care utilizing clinical judgment based on the physical, psychological and emotional needs of the clients. Consult, refer and collaborate with other disciplines involved in total patient care. Document all activity using appropriate medical record forms. Participate in ongoing educational programs to maintain clinical skills and proficiencies. Skills
The required skills for this position include strong clinical assessment abilities, excellent communication, and organizational skills to manage patient records and ensure regulatory compliance. Proficiency in bilingual communication, and electronic health record (EHR) systems is beneficial for efficient patient care. Physical Demands
