Physical Therapist (PT)

for a

per diem to part-time

opportunity providing

home health visits

in the Columbia area. This role serves adult and geriatric clients across Richland and Lexington Counties in South Carolina. As a home health Physical Therapist, you will be a vital part of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to remain at home. Performing diagnostic tests and measurements related to mobility, strength, balance, and safety. Implementing wound care interventions using evidence-based dressings and topical agents. Developing and updating personalized care plans, including manual therapy, gait training, and balance exercises. Assessing and revising care plans in collaboration with nursing staff. Educating clients and families on rehabilitative activities to promote health and independence. Documenting all observations and interventions accurately using electronic medical records. Qualifications: Current South Carolina Physical Therapist (PT) license. Graduation from an American Physical Therapy Association-approved program. Home health experience preferred. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem-solving skills. Proficiency with computers; experience with EMR systems is a plus.
