RN Triage – Allina Health

Overview This is an outpatient clinic role in which the RN triage scope includes triage services, anti-coagulation, and medication refills. The RN will provide patient care support for centralized nursing programs, outpatient and home care services, and will work in an outpatient clinic setting.
Principle Responsibilities
Assessment: Collect, prioritize and synthesize comprehensive data relevant to the patient’s health or situation; involve the patient, family and other health care providers as appropriate.
Diagnosis: Analyze assessment data to determine nursing diagnoses; interpret information to identify patient needs by age, development, and culture; document and communicate diagnoses with the patient, family and health care team.
Outcomes Identification: Identify and establish individualized, measurable outcomes in collaboration with the patient and family.
Planning: Develop an individualized plan of care that considers patient characteristics and ensures continuity of care; incorporate evidence-based nursing practice within regulatory guidelines.
Implementation: Execute the plan safely and timely; utilize evidence-based interventions and coordinate care as appropriate; document interventions per guidelines.
Evaluation: Assess progress toward outcomes; revise diagnoses, outcomes, and plans as needed; involve patient, family and care team; document revisions.
Quality of Practice: Participate in quality improvement activities and apply available data to improve nursing practice and outcomes.
Education: Maintain current nursing knowledge; participate in educational activities and apply learning to practice.
Professional Practice Evaluation: Self-evaluate against professional standards; seek informal feedback from patients, peers and others.
Collegiality: Support peers, share knowledge, provide constructive feedback, and contribute to a positive work environment.
Collaboration: Partner with patients, families and others to achieve positive outcomes.
Ethics: Maintain an ethical, therapeutic nurse-patient relationship; advocate for patients and use resources to support ethical decisions.
Research: Integrate best available evidence into practice.
Resource Utilization: Use safety, effectiveness, and cost considerations in planning and delivering care; manage resources responsibly.
Leadership: Provide leadership in the professional practice setting and promote a positive work environment.
Environmental Health: Practice in an environmentally safe and healthy manner and communicate health risks and prevention strategies.
Special Roles (as applicable): Charge Nurse or Preceptor duties when acting in those roles, including coordination of unit operations and orientation/mentoring as appropriate.
Other duties as assigned.
Required Qualifications
Associate's or Vocational degree in nursing
Minimum 3 years RN experience
Preferred Qualifications
Experience in triage, anticoagulation, or remote nursing support
Licenses/Certifications
Licensed Registered Nurse – Minnesota Board of Nursing
Licensed Registered Nurse – Wisconsin Department of Safety & Professional Services required by completion of orientation
Physical Demands
Sedentary work with occasional lifting up to 10 lbs.
Fully remote position, including orientation, with occasional onsite requirements
Must live within 60 minutes of Apple Valley, MN
0.80 FTE (64 hours per two-week schedule)
8 hour day shifts; every 3rd weekend and holiday rotation
Cross-trained to process medication refills
Pay Range Pay Range: $39.19 to $53.72 per hour. The base pay reflects factors such as experience, education and union agreements (if applicable). Shift, weekend and other differentials may apply.
