Overview
CenterWell Home Health is hiring a Full-time Physical Therapist. You’ll play a vital role in helping patients regain strength, mobility and independence—all from the comfort of their homes. By delivering personalized care that focuses on rehabilitation and functional improvement, you'll empower individuals to overcome physical limitations and improve quality of life. Compensation
Base pay range: $85,400.00/yr - $117,500.00/yr. This range is provided by CenterWell Home Health. Your actual pay will be based on your skills and experience — talk with your recruiter to learn more. Pay per visit/unit: $54.00 - $76.00 Responsibilities
Plan and administer prescribed skilled physical therapy treatment and training for patients with various injuries, illnesses and functional disabilities to attain the highest level of physical function. Test/screen the patient’s physical strengths to assist the physician in evaluating function and document findings to develop or pursue treatment programs with measurable training objectives. Develop/implement a conditioning/rehabilitation program aligned with the physician’s Plan of Treatment and the patient/rehab team goals; adjust treatment as needed to maximize results. Confer with physician and clinical team to obtain patient information and assist in developing, implementing and revising the therapy Plan of Treatment. Provide guidance to Physical Therapy Assistants and Home Health Aides with written instructions/care plans reflecting current plan of care; supervise/evaluate staff performance and ensure documentation is completed appropriately. Document patient care observations, interventions, and evaluations accurately and promptly. Ensure interim orders are documented, submitted for physician signature, and implemented. Report patient progress to the physician, Clinical Manager, staff, patient and family; submit evaluation, treatment plans, progress reports and discharge summaries to supervisors and care management staff. Qualifications
Degree from an accredited Physical Therapy Program (approved by the APTA) Minimum of one year physical therapy experience preferred Current and unrestricted Physical Therapy license Current CPR certification Strong organizational and communication skills A valid driver’s license, auto insurance, and reliable transportation are required Requirements & Benefits
