$85,400.00/yr - $117,500.00/yr Responsibilities

Plan and administer prescribed skilled physical therapy treatment and training for patients to attain highest level of physical function. Test/screen the patient’s physical strengths to assist the physician in evaluating function and document findings to guide treatment planning and measurable objectives. Develop/implement conditioning/rehabilitation programs aligned with the physician’s Plan of Treatment and patient goals; adjust treatment as needed. Collaborate with physician and clinical team to obtain information and revise the therapy plan as needed. Provide instructions and care plans to Physical Therapy Assistants and Home Health Aide staff; supervise/evaluate performance and monitor documentation. Document care observations, interventions and evaluations promptly and accurately; coordinate orders and signatures with physicians. Report patient progress to the physician, Clinical Manager, staff, patient and family; submit evaluations, treatment plans, progress reports and discharge summaries as required. Required Experience/Skills

Degree from an accredited Physical Therapy Program (approved by the APTA) Minimum of one year physical therapy experience preferred Current and unrestricted Physical Therapy license Current CPR certification Strong organizational and communication skills Pay Range
