Overview

The Wellness Nurse is responsible for monitoring the health and well-being of residents and provides leadership support and assistance with resident care and services. Responsibilities include overall well-being of residents, training and monitoring of medication care administration, communication with resident support services and families, and maintaining clinical quality assurance while leading Sunrise Senior Living’s mission to champion the quality of life for all seniors in accordance with applicable standards and Sunrise policies.

Responsibilities & Qualifications Essential Duties

Coordination of Health Needs: Identify ongoing needs and services of residents to promote the highest quality resident services; ensure proper follow-through, evaluation, and documentation for changes in health care services and status.

Assist Resident Care Director (RCD) in conducting resident evaluations and data gathering at move-in, 30 days post move-in, semi-annually, and with any status change.

Conduct monthly wellness visits for all residents.

Monitor and follow up with residents who have experienced a fall, accident, or incident; document and report as required.

Document all pertinent information in the resident wellness file and maintain communications with residents’ families or responsible parties to ensure medical needs are met.

Contact residents’ attending physicians as necessary or upon family request.

Ensure weights and vital signs are obtained monthly for each resident.

Maintain wellness files in compliance with Sunrise policies and federal/state/provincial regulations.

Maintain medical supplies and emergency kits for the community; provide clinical support to team members as needed.

Follow infection control practices and partner with RCD and community team to promote infection control standards, including resident testing, training, and skills checks.

Practice proper methods for attending to and disposing of (and exposure to) bloodborne pathogens, bodily fluids, infectious waste, sharps, and hazardous materials in compliance with universal precautions.

Medication Programs: Monitor each resident’s medication and treatment profile to ensure administration as ordered and documented accurately.

Transcribe physician orders into MAR/TAR with initials, date/time, and timely fax to pharmacy.

Complete MAR/TAR audit each month.

Demonstrate knowledge of the Six Rights of Medication Pass and act as liaison for pharmacy services.

Monitor Medication Room, carts, and treatment carts for neatness, stock, and expired medications.

Complete medication observations and skills checks for medication care managers as directed by the RCD.

Quality Assurance and Regulatory Compliance: Demonstrate knowledge of quality improvement areas (Resident Centered Care Model, APIE, Quality Care Indicators and Outcomes) and participate in Sunrise Quality Assurance and Clinical Meetings.

Demonstrate knowledge of regulations (federal, state/provincial, local), Resident Rights, and the Resident Assessment/Evaluation process in accordance with regulatory requirements.

Review physician reports of new residents as directed by the RCD; order medications and equipment as directed by the RCD.

Demonstrate knowledge of Sunrise practices and guidelines in resident care programs (skin, nutrition, behavior, falls, bowel/bladder) and assist in implementation, training, and monitoring of documentation related to residents’ clinical health and wellness.

Ensure changes, concerns, and solutions are reported to the RCD as appropriate; contribute to engagement programs and attendance at meetings.

Maintain compliance with required training and regulations applicable to this role.

Perform other duties as assigned.

Experience and Qualifications To perform this job successfully, the individual must be able to perform each essential duty satisfactorily. The requirements listed reflect the knowledge, skills, and abilities required.

Graduate of an approved college/school of nursing

Current state/provincial license as LPN/LV N

Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care

Knowledge of nursing practices, techniques, and methods applied to health and wellness resident care and regulatory requirements

Good assessment skills

Proficiency in computer skills, Microsoft Office, and Sunrise applications with ability to learn new applications

Ability to work weekends, evenings, and flexible hours as needed
