Child and Adolescent Mental Health Case Manager II (Bilingual - English/Spanish Speaking) at Washburn Center For Children. Responsibilities

Manage a caseload of 17-18 children or youth and their families with all client service expectations that accompany this caseload. Provide culturally based, individualized targeted Children’s Mental Health Case Management to children/students and families in their home, schools and community based on each family’s needs. Coordinate mental health and other health related services to improve or maintain emotional and behavioral functioning at home and in the community, help the family access resources (including third-party insurance or Medical Assistance), and reduce risk of out-of-home placement, maltreatment, or need for protective services. Complete timely documentation and planning including Individual Family Community Support Plans, Progress Notes, case management authorizations, and treatment plans. Day-to-day activities

Clinical Treatment: Educate and support families to become empowered to self-advocate and navigate systems; provide related services. Assessment: Complete a functional and diagnostic assessment of medical, social, educational, and other services needed to address mental health needs. Service Plan Development: Develop an Individual Family Community Support Plan (IFCSP) based on the functional assessment and diagnosis. Coordinate, refer, and monitor services for each case as applicable; ensure Minnesota and HIPAA data privacy requirements are followed. Education, Licensure & Skills

Must have a Master’s degree in Social Work, Counseling or Psychology If candidate has a Master’s in Social Work, valid licensure as an LGSW is required Fluency in English and Spanish A minimum of one year of experience with children and families in acute mental health distress is required Flexibility, resourcefulness, and demonstrated cultural responsiveness when working with diverse populations, specifically those impacted by systemic inequality Knowledge of community resources and experience with home and community-based services Must have vehicle in operating condition, a valid driver’s license, and car insurance Base pay range : $52,000.00/yr - $58,000.00/yr All are welcome!

Washburn Center’s case managers work collaboratively with families and across multi disciplines internally, to develop a care plan and ensure access to needed services, including mental, social, educational, health, vocational and recreational assistance. Our case managers advocate on behalf of the child and family’s needs and coordinate care across multiple providers. These services are provided for children age 3 through 17. Benefits at Washburn:
