CenterWell Home Health is hiring for the Physical Therapist, Home Health role. Base pay range is $85,400.00/yr - $117,500.00/yr. Become a part of our caring community and help us put health first. As a therapist at CenterWell Home Health, you’ll support patients in their homes to regain strength, mobility and independence through rehabilitation and functional improvement. Responsibilities
Plan and administer prescribed skilled physical therapy treatment and training for patients with injuries, illnesses or functional disabilities to attain the highest level of physical function. Screen and test patients’ physical strengths to support physician evaluation, document findings, and establish measurable training objectives. Develop and implement a conditioning/rehabilitation program aligned with the physician’s Plan of Treatment and patient/rehab team goals; adjust treatment as needed to maximize results. Collaborate with physicians and clinical team members to obtain patient information and adjust the therapy program and Plan of Treatment. Provide supervision and written instructions for Physical Therapy Assistants and Home Health Aides, reflecting the current plan of care and monitoring documentation completion. Document patients’ care observations, interventions and evaluations promptly and thoroughly; ensure orders from the physician and therapist are documented, submitted for signature and implemented. Communicate patient progress to the physician, Clinical Manager, staff, patient and family; submit evaluations, treatment plans, progress reports and discharge summaries as required. Qualifications
Degree from an accredited Physical Therapy Program (approved by the APTA). Minimum of one year physical therapy experience preferred. Current and unrestricted Physical Therapy license. Current CPR certification. Strong organizational and communication skills. A valid driver’s license, auto insurance, and reliable transportation are required. Pay and Schedule
