Overview
Physical Therapist (PT) - Home Health - Solano County ABL Health Care is seeking a compassionate, organized Physical Therapist (PT) to join our Home Health team as a full-time member in Solano County, CA. The role involves evaluating patients, developing individualized treatment plans, and improving mobility, strength, and quality of life in collaboration with the clinical team. The position supports care delivery both remotely and in the field.
Responsibilities
Provide physical therapy services within the scope of care, following the patient’s individualized plan, in coordination with the Interdisciplinary Team.
Administer treatments, monitor patient conditions, and report signs or changes to the Physician, Case Manager, and/or Director of Nursing as needed.
Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
Comply with all company policies and procedures to ensure high standards of care.
Qualifications
Current Physical Therapist (PT) License to practice in California – Required
Experience in Home Health or Hospice care – 1 Year Required
Current CA Driver's License
Able to travel within a 15–25 mile radius on a regular basis
