Position Overview:
As a PTA, you will evaluate patients’ physical health, develop individualized treatment plans, and work to improve their mobility, strength, and quality of life. You will collaborate closely with our Clinical Director and Clinical Staff, ensuring the highest standards of care while providing support both remotely and independently in the field.
Responsibilities
Provide physical therapy services within the scope of care, following the patient’s individualized plan and coordinating with the Interdisciplinary Team.
Administer treatments, monitor patient conditions, and report signs or symptoms indicating a change in condition to the Physician, Case Manager, and/or Director of Nursing as needed.
Maintain accurate clinical records and submit documentation per company policy using a company‑provided device.
Comply with all company policies and procedures to ensure high standards of care.
Requirements
Current Physical Therapist Assistant (PTA) License to practice in California (Required).
Minimum 1 year of experience in Home Health or Hospice care (Required).
Current California Driver’s License.
Ability to travel within a 15–25 mile radius on a regular basis.
