Summary
The clinical pharmacist is a licensed professional with knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics. Major Duties and Responsibilities of the Position Outpatient Pharmacy Functions
Manages the local prescription pending file, acting as a liaison and resolving problems as they arise.
Identifies cost‑saving opportunities by participating in formulary management activities.
Evaluates medication orders and profiles for therapeutic appropriateness.
Ensures safe, accurate, and appropriate distribution or dispensing of medications.
Inpatient Pharmacy
Verifies electronic orders for accuracy, performing prospective drug utilization evaluations with consideration of drug‑drug, drug‑allergy, and drug‑disease interactions.
Ensures indication is listed on all medication orders for patients admitted to any unit.
Minimizes therapy duplication and provides instructions for duplicate orders.
Guarantees proper labeling and data entry for unit dose packaging, including generic name, manufacturer, lot number, control number, and shipment number.
Clinical Functions
Conducts comprehensive patient assessments, completing medication histories and patient interviews.
Reconciles medications, updating profiles to reflect accurate VA and non‑VA drug lists.
Communicates orally and/or in writing with other health professionals regarding drug therapy selection and management.
Monitors drug therapy outcomes, recommending changes to providers and documenting in CPRS.
Sterile Preparation of Intravenous Admixture Solutions
Applies aseptic techniques in sterile solution preparation.
Performs scheduled cleaning and training to maintain USP 797 and 800 compliance.
Oversees and double‑checks all technician IV preparations.
Controlled Substances
Resolves inventory discrepancies prior to dispensing and reports findings promptly.
Fills controlled substances in both inpatient and outpatient settings per established policies.
Distributes orders and requests for controlled substances following pharmacy procedures.
Follows VHA, DEA, and pharmacy policies and informs the Chief of Pharmacy Service of discrepancies.
Inventory
Applies advanced inventory principles, coordinating with procurement tech and Chief of Pharmacy Service to manage levels.
Receives supplies, identifying shortages, tampering, breakage, deterioration, or expiration, and selects proper storage.
Equipment
Operates and maintains complex pharmacy equipment such as ScriptPro, unit dose packaging machines, and inpatient automation.
Uses equipment following manufacturer, facility, and service policies and procedures.
Academic/Teaching/Training
Provides leadership, guidance, and consultation to staff across the Medical Center and community clinics.
Maintains licensure and professional development to stay current with practice and Joint Commission standards.
Work Schedule Monday – Friday, 7:00 a.m. – 3:30 p.m.

Basic Requirements
United States Citizenship.
English language proficiency: spoken and written proficiency required.
Education: Graduate of an ACPE‑accredited College or School of Pharmacy with a BS Pharmacy or Pharm.D. Degree; foreign graduates must provide FPGEE and TOEFL iBT certification.
Licensure: Full, current, unrestricted license to practice in a U.S. jurisdiction; temporary graduate appointments may be made under authority 38 U.S.C. § 7405(c)(2)(B) for up to 2 years.
Grandfathering Provision for current VHA employees may apply.
Grade Determinations
GS-11 Pharmacist (Entry Level) : No experience or education beyond basic requirements.
GS-12 Clinical Pharmacist (Full Performance Level) : Must meet one of the following:
One year of experience equivalent to next lower grade level.
Completion of an ACPE‑accredited Pharm.D. program.
Additionally, must demonstrate KSAs:
Knowledge of professional pharmacy practice.
Oral and written communication skills with patients and staff.
Knowledge of laws, regulations, and accreditation standards related to drug distribution and pharmacy security.
Skill in monitoring and assessing drug therapy outcomes, including laboratory interpretation.
Preferred Experience
1 or more years of experience.
Active U.S. State Pharmacist License.
Strong clinical judgment and decision‑making skills.
Completion of an ACPE‑accredited Pharm.D. program.
PGY1 Residency.
Physical Requirements Must be physically and mentally able to perform essential functions without hazard to self or others. Requirements include visual acuity, hearing, speech, manual dexterity, ability to reach above shoulder level, lift 15–44 pounds, sit, stand, and walk for long periods. Must be flexible and able to handle stressful situations.
