Full-Time | 8:00 AM - 5:00 PM Salary:

Up to $74,880/year (commensurate with experience) Live Oak Healthcare and Rehabilitation is seeking a dedicated Social Services Director to join our leadership team. In this vital role, you will ensure residents and their families receive the emotional, social, and mental support they need while guiding care in accordance with state and federal regulations. As a key leader in our interdisciplinary team, you will oversee social service programs, lead discharge planning, and strengthen community partnerships—helping residents thrive and families feel supported every step of the way.

Responsibilities

Lead and coordinate resident discharge planning, financial assistance, and community referrals.

Ensure residents receive behavioral health services that align with individualized care plans.

Facilitate resident and family council meetings; educate on resident rights, advance directives, and available resources.

Maintain compliance with state and federal regulations; recommend and implement policy updates.

Provide translation support and culturally sensitive guidance for residents with limited English proficiency.

Maintain a comprehensive resource library for regulatory and compliance materials.

Collaborate with all departments to enhance resident quality of life and satisfaction.

Support facility surveys, inspections, and quality improvement initiatives.

Build and nurture partnerships with community agencies to expand resident support services.

Qualifications

Minimum 3 years of supervised social work experience in a healthcare setting, preferably skilled nursing.

Strong leadership, collaboration, and team-building skills.

Compassionate, patient, and adaptable to meet the diverse needs of residents and families.

Knowledge of state and national licensing and accrediting requirements.

Clear and professional communication skills.

Commitment to maintaining a safe and supportive environment for residents and staff.
