APP Hospitalist (SHIFT) Responsibilities

Provide assistance to the Hospitalist service in gathering and validating daily patient data (vital signs, ancillary reports, intake/output reviews, and others). Perform patient rounds with the Hospitalist and independent rounds when appropriate; identify patients to be discharged; ensure timely execution of Hospitalist orders. Offer input to patient care decisions and planning; act as a liaison between physicians and case management for discharge planning needs. Assist in facilitating timely discharges (transcribing discharge orders, preparing prescriptions, confirming discharge plans with family members and Case Management). Instruct patients/significant others and health care professionals regarding disease process, treatment plan, and admission through discharge needs. Utilize evidence-based medicine to improve care and outcomes for Hospitalist patients. Work full time with coverage for vacations, weekends, and holidays. Assist in development/review of policies and procedures; serve as a resource to staff in applying policies; advocate for patients, families, and healthcare professionals. Prioritize patient rounds to facilitate timely discharge and address current medical needs; coordinate communication with primary physicians and care teams regarding dictations, pending lab/x-ray results. Contact patients within 24 hours of discharge to review discharge plan, medications, follow-up, and address questions; assist other agencies with disposition needs as they arise. Collaborate with physicians and health care team to discuss and resolve patient care issues and ensure quality outcomes. Assess patients’ physical and psychosocial status to plan and implement safe, effective care plans; develop and review policies, guidelines, protocols, and standing orders under the direction of the Hospitalist Medical Director. Establish ongoing relationships with service providers; participate in professional activities related to the role. Skills and Abilities

Willingness to learn new roles within scope of practice. Ability to develop and maintain effective working relationships. Strong organizational skills for timely patient appointments and follow-up. Courteous, service-oriented communication with patients, families, physicians, and co-workers. Participation in NP/PA activities within Prevea and the community. Participation in community activities; ability to develop coordinated programs for populations. One year of hospital APP experience preferred. Education/Experience/Certifications

ADVANCED NURSE PRACTITIONER Registered nurse in the State of Wisconsin. Graduate of a master’s level nurse practitioner program. Certification through ANCC or ANA. Certified in the State of Wisconsin as an Advanced Practice Nurse Prescriber or certification eligible, obtaining certification at the first opportunity. Maintains current CPR certification; ACLS certification desirable. Experience requirements may vary. Maintains current CPR certification, ACLS certification desirable. Experience requirements may vary. PHYSICIAN ASSISTANT Graduation from an accredited physician assistant program. Licensed by the State of Wisconsin. Certification from the National Commission on Certification of Physician Assistants. Maintains current CPR certification; ACLS certification desirable. Experience requirements may vary.
