The salary range for this position is $34.28 - $46.28 (Hourly Rate). Placement within the salary range is dependent on relevant work experience and internal equity. For positions represented by a labor union, placement within the salary range is guided by the rules outlined in the collective bargaining agreement.

Description The Social Worker LSW reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

Responsibilities

Psychosocial Assessment and Intervention

Meets directly with patient and family to perform a comprehensive assessment including social, emotional, cultural, mental status, environmental and financial circumstances in conjunction with interdisciplinary assessment of the patient.

Recommends a plan of intervention based on patient needs, preference and mutually established goals.

Provides psychosocial interventions including reactions to illness and disability, especially the chronically and terminally ill. Facilitates informed decision making (including advanced directives) and development of treatment and intervention plans. Supports adjustment to the hospital setting and adherence to the treatment plan. Addresses post-hospital care needs and links to community resources. Includes gynecological/obstetrical issues and related topics as needed. Addresses issues related to insurance coverage and payment, psychiatric symptoms, chemical dependency, conflict resolution, and family dynamics impacting care and discharge plans.

Performs assessments of the physical environment and adequacy of support systems for outpatients to prevent a crisis and/or hospitalization.

Provides crisis intervention and/or Protective Services for specified at-risk populations and coordinates guardianship/protective services as needed.

Manages Discharge Planning through Placement Coordination, Resource Utilization, and Coordination of Skilled Home Health Care.

Participates in discharge planning stages and ensures the plan of care is coordinated, communicated, and implemented with the team, patient, and family.

Provides initial screening for new patients to assess medical necessity, funding sources, and likelihood of needing Social Work/discharge planning services.

Serves as the point person for the plan of care related to discharge planning through communication and collaboration in rounds, conferences, and multidisciplinary discussions.

Coordinates action plans to resolve barriers and facilitates timely discharge through early identification of post-hospital care needs.

Ensures and maintains plan consensus among patient/family, physician, and payer.

Engages in ongoing discharge planning activities with the multidisciplinary team and proactively resolves delays.

Uses advanced conflict resolution skills to ensure timely resolution of issues and system problems.

Seeks consultation and makes referrals to appropriate disciplines to expedite discharge planning.

Demonstrates knowledge of community resources and connects patients and families with these resources.

Advocates for patients to gain access to needed information, resources, or services.

Facilitates review of high-risk cases and communicates needed interventions with the team.

Coordinates interventions with the healthcare team and ensures successful outcomes.

Provides patient and family education to promote wellness and understanding of the health care system.

Demonstrates knowledge of the utilization management process including level of care assignments and payor communication.

Participates in Clinical Performance Improvement activities and contributes to data collection and reporting of metrics.

Uses data to drive decisions and plan/implement performance improvement strategies for assigned patients/units.

New graduates are required to participate in weekly clinical supervision with an LCSW until a minimum of 3000 supervised hours are fulfilled.

Upon completion of three years post-master9s degree, eligible to provide graduate level Social Work field supervision requiring a field placement.

Maintains professional development and CEU requirements through workshops, conferences, and in-services.

Complies with Northwestern Memorial Hospital policies on patient confidentiality including HIPAA requirements.

Qualifications

Masters Degree in Social Work from a school of Social Work accredited by CSWE.

A high level of interpersonal skills to affect positive outcomes.

Organizational skills to manage an appropriate caseload and perform Social Worker functions.

Self direction required for daily work.

Analytical skills to collect, analyze, interpret data, solve problems, and negotiate in sensitive situations.

Licensure in Illinois. Licensed Social Worker, LSW.
