The TAY Health and Wellness Center is located at: 888 Post Street, San Francisco, CA.

Position Summary The TAY HWC Case Manager outreaches to, intakes, assesses, counsels, and coordinates care for TAY ages 18-27 years old to achieve safety, health, and well-being across multiple life domains including housing/shelter, education and employment, behavioral health, physical health, community building, family and relationships, and more. The Case Manager serves as the client’s main advocate and care coordinator within GLIDE’s continuum of care, community partners and resources, and other external systems of care. This position works with other TAY HWC team members to enable warm hand-offs and linkages among specialists, case managers, navigators, and ambassadors. Upon exit, develops a care transition plan to ensure clients are linked to additional support services. In addition to 1:1 case management, this role will facilitate and/or coordinate weekly activities or psychoeducation groups to support clients’ confidence, wellness, community connection, and growth mindset. Working hours may include nontraditional hours and some weekend work.
Essential Duties And Responsibilities
Initiate and conduct outreach with TAY at the TAY HWC Center and offsite locations where TAY may access services or congregate for population engagement.
Administer intake and assessments to enroll TAY into GLIDE’s continuum of care, establish baseline safety and health needs, and develop individual goal/service plans with actionable steps for care coordination.
Develop a care coordination plan based on 1:1 sessions, referrals, and assessments; identify key care coordination needs within GLIDE’s integrated services and establish successful linkages to care with GLIDE partners (e.g., UCSF, Street Medicine, Clean Slate, Larkin St.).
Support TAY to navigate and overcome barriers related to safety, health, and economic stability by teaching and coaching self-advocacy and effective communication.
Identify knowledge and skills gaps to support ongoing stability and link TAY to educational resources addressing topics such as sexual health, personal hygiene, harm reduction, relationships, literacy, digital literacy, and financial literacy.
Coordinate, facilitate, and/or conduct expressive arts activities, therapeutic activities, or psychoeducation groups to support confidence, wellness, community connection, and growth mindset.
Monitor client progress through regular appointments and follow-ups; maintain accurate records of intake, assessments, progress, and outcomes in required databases and files.
Participate in staff and community meetings to build relationships with GLIDE programs, community partners, and City agencies to enhance service delivery.
Minimum Qualifications
Bachelor’s degree in social work, public health, community health, or related field preferred.
5+ years of experience in providing intensive case management to TAY or similarly disenfranchised, high-need populations in social service settings.
Experience and demonstrated success providing 1:1 counseling and case management.
Knowledge of Bay Area SF TAY needs and resources, especially those experiencing poverty, homelessness, mental health challenges, substance misuse, trauma, and related experiences.
Experience facilitating weekly activities or psychoeducation groups that support client confidence, wellness, and community connection.
Ability to work independently, demonstrate initiative, reliability, and consistency.
Ability to collaborate with internal and external stakeholders to coordinate care for shared clients.
Strong written and oral communication skills; excellent interpersonal skills.
Attention to detail, strong organizational, time management, and problem-solving skills.
Proficiency with computer systems including Windows, Microsoft Office Suite, Salesforce or similar databases, Zoom, and internet research.
Work Environment
GLIDE’s buildings are located in the Tenderloin neighborhood.
GLIDE maintains an open-door policy to the community and its clients.
Physical Requirements
Ability to work on a computer and see details at close range.
Ability to hear within normal range and communicate effectively (in person, telephone, or Zoom).
Finger dexterity and the ability to use standard office equipment.
Sit or stand for extended periods; ability to navigate office spaces (via elevator or stairs).

$60,000 - $70,000 per year
This is a full-time (40 hour/week) non-exempt position.
This role is covered by the collective bargaining agreement.
