Position Overview: We are seeking an experienced Dialysis Clinic Intake Manager to lead our intake operations and enhance the patient experience at our clinics. The ideal candidate will bring a strong background in case management and social work, along with a passion for serving the community. In this role, you will be responsible for overseeing the intake process, coordinating patient care, and collaborating with our healthcare team to ensure the highest level of service. Key Responsibilities:
- Manage the daily operations of the intake department, ensuring optimal patient flow and service efficiency. - Conduct comprehensive assessments of new patients to determine their needs, eligibility, and appropriate care plans. - Facilitate communication between patients, families, and the clinical team to address concerns and provide support. - Ensure compliance with all regulatory standards and organizational policies. - Train, mentor, and supervise intake staff, promoting a collaborative and patient-focused culture. - Monitor and analyze patient intake data to identify trends and areas for improvement.
Qualifications:
- Bachelor's degree in social work, nursing, healthcare administration, or a related field (master's degree preferred). - At least 5 years of experience in a healthcare setting, with a strong emphasis on case management or social work. - Familiarity with dialysis procedures and patient care processes is highly desirable. - Exceptional communication and interpersonal skills to build rapport with patients and families. - Proven leadership experience with the ability to manage and motivate a team. - Strong organizational skills and keen attention to detail. - Proficient in electronic health records (EHR) and healthcare management software.
